fix(write-to-file): capture streaming failure once, report it once (split 3/6 of #1066) - #1930
U5 — streaming failure capture + single error reporting
Part of the upstream PR 1066 split. Own issue: 1935. Content source of record: 72143527fd33306e5541116093c2cbf803cce9e0..pr-1066-audit).
Why this unit exists: handlePartial captures the streaming failure once and reports it once - no duplicate error bubble; the authoritative execute() error is the one surfaced.

src/core/tools/__tests__/writeToFileTool.spec.ts: OK (content subset of source)

issue: the split plan
Chain position
Merge order is fixed: U12 (1927) -> U4 -> U5 -> U3 -> U6 -> U7 -> FINAL (1928). This PR is opened against main because the split branches live on the fork; the diff GitHub shows is therefore cumulative through this unit. The unit's own content is the delta from the previous unit head (U4), listed under Fidelity above. The sole merge target of the series is the FINAL integration PR (1928); merging the chain in order keeps every bot-visible diff clean.
ESLint: clean on every touched file; suppression counts unchanged
No .changeset file, no CHANGELOG edit.
Recreate policy
If the bot stalls on a pre-merge check and the existing head cannot obtain bot review/approval (empty-commit re-trigger attempted and failed), the unit is recreated from the tagged content source of record — never from a per-PR head. At most 1 PR per issue.

Round update — Lifecycle Resource Cleanup + regression evidence for the rollback this unit changed
Shared root cause behind the Lifecycle Resource Cleanup row (all five units of 1066).handlePartial() registers this task's partial-stream entry — and its TaskAborted listener — before it checks the prevent-focus-disruption experiment. With the experiment enabled the delta returns without ever showing a preview and never reaches execute()'s teardown, so the entry and the listener stay attached for the rest of the task's life, and a streamFailed mark armed by an earlier failed delta keeps suppressing this task's later diff previews. The sibling units carry the same release in their own PRs, each verified red-first with a negative control.
This unit (U5):execute() is covered by the finally block, so only the suppressed-preview return in handlePartial() lacked a release; it now has one.
Red first: the new test failed with expected 1 to be +0. Green: 40 passed / 5 skipped. Negative control: removing the release turns exactly that one test red; restored green.
Regression evidence for the revertChanges() rollback this unit changed, added at the owning layer (DiffViewProvider.spec.ts):
an existing-file revert with no activeDiffEditor stops at the guard instead of dereferencing it — negative control: removing the guard makes the call reject with TypeError: Cannot read properties of undefined (reading 'document'), i.e. the guard is observably what stops it;
a created directory that never landed (rmdir ENOENT) does not abort the remaining rollback — negative control: dropping the ENOENT tolerance turns exactly that test red;
a non-ENOENT rmdir failure still reaches the caller — negative control: widening the tolerance to swallow every error turns exactly that test red. Together these pin fail-closed behaviour against over-tolerance.
Main refresh. Merged org main 036245c5e (U1 1927). U1's content no longer appears in this diff: 11 files +1471/−40 → 8 files +1091/−35, 0 behind main. Conflicts were confined to src/core/task/__tests__/Task.spec.ts (and Task.ts on U7) — the region U1 rewrote; resolved by taking main's version of the shared save-stage tests (try/finally plus the fixed-task-id ui_messages.json cleanup from cf9206a42) rather than re-implementing U1.
Verification after the merge: Task.spec 172 passed, writeToFileTool.spec 40/5, DiffViewProvider.spec 76 passed / 0 failed (the saveChanges default-values failure that was red locally on this branch is fixed by main), eslint 0/0.
Resolve them on the pull request, or post @coderabbitai approve as a new top-level comment to resolve them and request approval.

Bug Fixes
Interrupted or malformed file writes now clean up temporary state, helping prevent stale previews or partial edits from affecting later writes.
Failed edits attempt to restore original files and remove newly created files and folders. If restoration fails, the failure is reported and further writes are blocked until recovery succeeds.
Previews reset after write errors, and cancelled tasks skip later preview updates.
Recovery avoids overwriting newer edits. Malformed or incomplete write requests report relevant write or restoration errors and clean up active edit state.
The change adds task-scoped cleanup and failure handling for partial write_to_file calls. DiffViewProvider reports rollback outcomes and supports recovery of streamed edits. Malformed calls and task lifecycle paths release stream state or rollback markers.

Summary
Diff rollback and filesystem cleanup src/integrations/editor/DiffViewProvider.ts, src/integrations/editor/__tests__/DiffViewProvider.spec.ts
revertChanges() now reports whether rollback completed. Rollback restores and saves buffers before closing tabs or removing new-file artifacts. Removal ignores ENOENT and propagates other filesystem errors. Recovery can reacquire the editor using captured rollback context.
Per-task stream state and write execution src/core/tools/BaseTool.ts, src/core/tools/WriteToFileTool.ts, src/core/tools/__tests__/writeToFileTool.spec.ts, src/core/tools/__tests__/writeToFileTool-partial-state-cleanup.spec.ts
BaseTool adds a parse-failure teardown hook. WriteToFileTool manages task-scoped stream state, cancellation, partial-ask finalization, rollback failures, and execution cleanup. Tests cover error handling, task isolation, cancellation, and refused-rollback recovery.
Malformed write_to_file calls release stream state and select tool-result content based on teardown results. Task request, disposal, and failed-history cleanup paths clear task state or rollback markers.

Explanation
The title names the main change: streaming failure in write_to_file is captured once and reported once. The changeset matches this. WriteToFileTool.ts adds per-task partial-stream state and single-report logic, and the tests cover it. The title is 87 characters, which is longer than the short-title guidance, and it includes a split reference '(split 3/6 of #1066)' that adds process noise. Those points do not change the verdict because the title still describes a real primary change.
Full details: Description check
Explanation
The description follows the template's main sections: linked issue (Closes #1935), description of the change and its rationale, and verification results. It omits the template's Pre-Submission Checklist, Visual Snapshots, Videos, Documentation Updates, and Get in Touch sections. It also contains a large amount of process content (merge chain, recreate policy, round updates) that the template does not ask for. The Description section is thin: it gives little explanation of the implementation, such as the per-task state design, rollback-refusal handling, or the editSession check in DiffViewProvider. The raw summary shows the change set is much broader than 'capture once, report once'. Core content is present, so the check passes, but the author should complete the template sections and add a clearer implementation summary.

Explanation
[#1935] Requirement: handlePartial() captures the streaming failure once, reports it once, and surfaces the authoritative execute() error without a duplicate error bubble.
Evidence from the whole-PR change summary (merge base 73931fe to head 6702cc0):
WriteToFileTool.ts keeps per-task partial-stream state (stabilized path, streamFailed, streamError). It skips later deltas after a streaming failure.
writeToFileTool.spec.ts asserts that streaming deltas do not create directories and do not report errors through handleError. Streaming-error expectations now assert capture.
Execution errors finalize the pending partial ask before reporting. Duplicate reports are suppressed.
The suppressed-preview return in handlePartial() releases its partial-stream entry and TaskAborted listener.
Limits:
The whole-PR diff exceeds the read cap. This result rests on the change summary and the PR description, not on full source.
The cross-phase test assigned to U6 is outside this requirement, per the issue deviation record.
The author reports a five-case Ubuntu failure in presentAssistantMessage-write-to-file-stream-cleanup.spec.ts. That spec covers captured stream failure reporting. Its status at head 6702cc0 is not confirmed. Tests were not run in this assessment.
Full details: Out of Scope Changes check
Explanation
[#1935] declares two files: src/core/tools/WriteToFileTool.ts (+38/-13) and src/core/tools/tests/writeToFileTool.spec.ts (+229/-3), 283 lines in total. The issue scope is streaming-failure capture and its single reporting point.
The whole-PR summary at head 6702cc0 lists 13 changed files. The changes to the declared files are much larger than the declared budget: WriteToFileTool.ts +651/-77 and writeToFileTool.spec.ts +1295/-11.
Changes with no concrete link to streaming-failure capture and single reporting:
ClineProvider.ts (+6) and removeClineFromStack-delegation.spec.ts (+42): failed-history cleanup for PendingActionSettlementError.
Task.ts (+26/-1), Task.dispose.test.ts (+7/-6), Task.spec.ts (+32): rollback-failure marker lifecycle, reversion-promise handling in disposal, and per-request write-tool state clearing.
BaseTool.ts (+33/-1), presentAssistantMessage.ts (+47/-1), presentAssistantMessage-write-to-file-stream-cleanup.spec.ts (+398): releaseStreamStateOnParseFailure and malformed-completion teardown.
DiffViewProvider.ts (+439/-26) and DiffViewProvider.spec.ts (+1433/-1): rollback recovery context and editSession, refusal handling, ENOENT tolerance, dirty-buffer restoration, and timed-out acquisition handling. The PR description's round update presents these as rollback work for this unit.
writeToFileTool-partial-state-cleanup.spec.ts (+291, new): cancellation paths and rollback-refusal tests.
Within WriteToFileTool.ts, the rollback-refusal records, recovery, releaseTaskRollbackFailure, clearTaskState, and resetPartialState additions are not required by the #1935 contract as written.
The PR description still states base 52699c6 and 8 files, +1091/−35. These do not match the reviewed merge base 73931fe or the 13-file whole-PR diff.
Resolution
Choose one of the following remediation paths.
(a) Keep only the streaming-failure capture and single-report logic in WriteToFileTool.ts and its tests, as #1935 declares. Move the other files and hunks to their owning units: ClineProvider.ts and removeClineFromStack-delegation.spec.ts; Task.ts, Task.dispose.test.ts, and Task.spec.ts; BaseTool.ts, presentAssistantMessage.ts, and presentAssistantMessage-write-to-file-stream-cleanup.spec.ts; DiffViewProvider.ts, DiffViewProvider.spec.ts, and writeToFileTool-partial-state-cleanup.spec.ts. Remove the rollback-refusal, recovery, releaseTaskRollbackFailure, clearTaskState, and resetPartialState additions from WriteToFileTool.ts unless the streaming-failure path requires them. Keep only the DiffViewProvider rollback calls that the streaming-failure path requires.
(b) Amend #1935 to name each extra file and state why it is required for the streaming-failure contract. Then move the rollback work that the issue does not require to its owning unit.
In either case, correct the PR description. Update the base commit, the file count, the line counts, the round-update rollback claims, and the boundary and scope statements so they match the reviewed merge base diff.
Full details: Security Boundaries
Explanation
src/core/tools/WriteToFileTool.ts:406-428 retries rollback before the .rooignore check at line 470. If a rollback fails and the user then adds the target to .rooignore, a retry can still restore and save or delete that target through DiffViewProvider.ts:626-723,733-803. The new recovery path bypasses the current access policy.
Resolution
Check the actual recovery target against the current access policy before recovery. If access is denied or the target is unknown, retain the refusal and require safe manual recovery. Add a regression test for a target added to .rooignore after rollback failure.
Full details: Persistence Integrity
Explanation
Rollback can delete another process’s file. After WriteToFileTool.ts:713-714 classifies a path as new, another process can create a read-only file in a writable directory. If placeholder creation fails, the new error handler calls rollback (WriteToFileTool.ts:780-807), which unlinks the path without checking ownership (DiffViewProvider.ts:563-565, 752-776). The base rollback returned when no editor existed.
Resolution
Create placeholders exclusively. Delete a file during rollback only if the edit still owns that file. Add regression coverage for concurrent creation before placeholder creation fails and for replacement before rollback.
Full details: Regression Evidence
Explanation
Request-lifecycle cleanup lacks focused coverage. Task.ts:4234 clears stale write-stream state before a new request; without it, the streamFailed guard at WriteToFileTool.ts:653 suppresses later previews. The request-flow test at Task.spec.ts:6606 does not seed or check that state. Other tests call clearTaskState() directly or cover malformed-call cleanup.
Resolution
Add a request-flow test that seeds stale streamFailed state and verifies that the next request clears it before model fetching or attemptApiRequest().
Full details: Lifecycle Resource Cleanup
Explanation
Three lifecycle issues remain: - WriteToFileTool.ts:668-674 retains an unstabilized delta in a singleton map. Direct Task.dispose() removes listeners but does not clear that map (Task.ts:3700-3708, 3758-3760), so the task and its provider remain reachable. - If rollback fails after disposal, WriteToFileTool.ts:801-824 adds a refusal record after disposal has cleared it. The record can outlive the task. The parse-failure rollback has the same race at lines 337-347. - If disposal occurs while new-file open() awaits directory creation, the changed rollback can accept a missing placeholder and reset the provider (DiffViewProvider.ts:733-780, 824-827). The pending open() can then create the file and directories (DiffViewProvider.ts:172-176). The reset prevents a later rollback from removing them.
Resolution
Clear per-task stream state during direct disposal. Prevent in-flight rollback failures from recording state after disposal. Coordinate disposal with pending opens and remove resources created after cancellation. Add regression tests for these races.

The only caller in this unit is the streaming-failure cleanup, whose error is reported by the authoritative non-partial block in execute() (see the comment above that call site), so porting the user-visible report here would double-report the same failure. No code change is made on this branch; the helper's new return value is simply unused here.

Persistence Integrity (pre-merge check) — fixed at 7752664
Confirmed: DiffViewProvider.open() creates the parent directories (createdDirs, line 130) and writes an empty placeholder (line 134) BEFORE it awaits openDiffEditor() (line 173). If that await rejects there is no activeDiffEditor, and revertChanges() returned at its 'if (!this.relPath || !this.activeDiffEditor)' guard — so the placeholder and the new directories stayed on disk, reset() then cleared the createdDirs tracking, and the next execute() saw an empty file and treated the requested new file as an existing one (editType modify), which a denial preserves. handlePartial()'s streaming-failure cleanup, the new code in this unit, calls exactly that rollback, so the path is reachable.
The filesystem rollback (unlink plus reverse rmdir) now runs regardless of whether an editor exists; only the document work (save a dirty buffer, closeAllDiffViews, closeFileTab) requires one.
removeCreatedFile / removeCreatedDir tolerate ENOENT: the failed open may never have written the placeholder, and that must not abort the rest of the cleanup.
The second half of the finding follows from the first: with the placeholder removed, execute() sees no file and takes the new-file path, so a rejection removes it instead of preserving an empty file. No change to execute() was needed.
Tests (DiffViewProvider.spec — the layer that would have failed): 'revertChanges() removes the placeholder and created dirs when open() failed before the editor existed' (unlink for the relPath, rmdir in reverse order) and 'revertChanges() tolerates a placeholder that was never written' (ENOENT does not propagate, delete still attempted). Pin: restoring the old early return fails BOTH tests.
Local: integrations/editor + writeToFileTool.spec + core/task + assistant-message = 892 passed; the single remaining failure (saveChanges default delay) reproduces without these changes. tsc --noEmit 0; eslint 0 err / 0 warn on both files.
Still open from the checklist: Regression Evidence (the captured streamError has no reader in this unit) and the Lifecycle Resource Cleanup warning (per-task state and the TaskAborted listener survive a parameter-parse failure and the early returns in execute()).

Regression Evidence + Lifecycle Resource Cleanup — addressed at 753c22f
The capture had no reader: streamError was stored for one reason (when the finalized block never reaches execute(), execute()'s authoritative filesystem retry never happens, so the captured error is the only failure the user can act on), but the teardown hook only released state and BaseTool still reported the incidental parse error. The doc block promised reporting with execute()'s 'writing file' context and suppression of the parse error; the body did neither.
clearTaskStreamState becomes releaseStreamStateOnParseFailure(task, callbacks): releases THIS task's state, restores the diff document the stream opened (revert before reset, like the other teardowns), and when a streamError was captured reports it with the same 'writing file' context execute()'s catch uses, returning true so BaseTool suppresses the generic parse error. The failure surfaces exactly once.
With no captured error the generic parse error is still reported (the pre-existing test still passes); other tools keep the default hook returning false.
execute()'s early returns (missing path, missing content, rooignore denial) returned before the success/catch cleanup, leaving the per-task state and its TaskAborted listener alive for the task's lifetime — and a retained streamFailed suppresses the diff preview of every later write_to_file in that task. They now release only this task's state.
Tests: 'reports the captured streaming failure once instead of the incidental parse error' (handleError called once with the captured error, not with the parse error; state released; revertChanges + reset ran) and 'releases the per-task stream state when execute() returns early on a denied path'. Pins: returning false instead of reporting fails the first; removing the release in the rooignore branch fails the second.
Local: core/tools + writeToFileTool.spec + assistant-message + core/task = 1420 passed / 5 skipped; tsc --noEmit 0; eslint 0 err / 0 warn on all three files.
Re-requested at head 753c22f: Regression Evidence and the Lifecycle Resource Cleanup warning are addressed (see the note above); Persistence Integrity was fixed at 7752664.
CI note on 9bbb2aea3, written from the job log rather than the check-run badge.
platform-unit-test (ubuntu-latest) failed on job 128528957375 (run 42414306193). It is not an infrastructure or teardown failure: the log has 0onUserConsoleLog lines, 0EnvironmentTeardownError lines and 0Unhandled rejections, and it ends with Test Files 1 failed | 197 passed (198) followed by ##[error]Process completed with exit code 1. The single failing file is src/core/assistant-message/__tests__/presentAssistantMessage-write-to-file-stream-cleanup.spec.ts, describe presentAssistantMessage - abandoned write_to_file stream, with 5 failing cases. Three signatures, quoted:
AssertionError: expected [ { type: 'tool_result', …(2) }, …(1) ] to have a length of 1 but got 2 at spec line 248 (expect(results).toHaveLength(1)), with is_error and toContain(STREAM_FAILURE) at 249-250,
AssertionError: expected "vi.fn()" to be called 1 times, but got 0 times at spec line 271 (expect(task.diffViewProvider.revertChanges).toHaveBeenCalledTimes(1)), with the reset count and the invocationCallOrder assertions at 272-273,
AssertionError: expected 1 to be 2 // Object.is equality on a third case in the same describe.
platform-unit-test (windows-latest) (128528957502) reads cancelled because the matrix fail-fasted after the ubuntu leg failed; it is not itself evidence of a second failure.
What is not being claimed here. The author's local gate set for this commit ran writeToFileTool.spec, writeToFileTool-partial-state-cleanup.spec, DiffViewProvider.spec and Task.spec (374 passed, 5 skipped) plus tsc --noEmit and the package lint script, and this spec was not among them, so the failure is not being described as platform-only until the spec has been run on the author's tree and checked for it.skipIf(process.platform === "win32") cases in it. If it is red locally as well, this is a real regression on the abandoned-stream path introduced by 9bbb2aea3, which adds DiffViewProvider.attemptRollbackRecovery as a second way into the provider and narrows the fail-closed refusal — exactly the region whose contract spec lines 269-273 state as revertChanges() running before reset().
One correction to the campaign's own record, so nobody misreads an earlier note. A comment posted earlier tonight about platform-unit-test (ubuntu-latest) described three observations in which the job was green with 0 onUserConsoleLog and 0 EnvironmentTeardownError (jobs 128487929229 on ec3589e8e, 128504640717 on 0625de6dd, 128512816982 on 0f5245802), and its subject was an abort-listener teardown signature. That note does not cover this failure, which has a different signature and no teardown markers at all, and it should not be cited as a non-reproduction of it. #1567's ubuntu leg went green on its merged head 24dee10cc without anyone re-running it; whether that failure and this one share a cause is not established, and 'the ubuntu job is flaky' would be two different problems wearing one job name until they are compared.
…r entry points the teardown now calls
The head this branch just pushed (9bbb2ae) left core/assistant-message/__tests__/presentAssistantMessage-write-to-file-stream-cleanup.spec.ts red - locally on Windows with the same signatures CI reported on ubuntu (5 failing cases: two tool_results where one is expected, revertChanges called zero times, and the open-count assertion that follows), and the windows matrix job cancelled by fail-fast. My gate set for that commit was writeToFileTool.spec, writeToFileTool-partial-state-cleanup.spec, DiffViewProvider.spec and Task.spec, and this file was not in it, which is how a head with a red required job got pushed.
The contract did not move, so no assertion moved. At 9bbb2ae the abandoned-stream teardown still runs captureRollbackRecoveryContext() at WriteToFileTool.ts:279, then revertDiffChangesBeforeReset() at :280 - which is the call that invokes revertChanges() - then resetDiffViewAfterWrite() at :287, i.e. revert before reset in the order the spec pins at its lines 308-314. What the commit added is a provider call in front of that revert, and this spec builds its Task as a structural double whose diffViewProvider literal only carried editType, isEditing, open, update, reset and revertChanges. The missing member threw inside the teardown before the revert could run, which is why revertChanges recorded zero calls, why a second tool_result appeared, and why the later open-count assertion drifted. The fix is the double, not the assertions: it now carries captureRollbackRecoveryContext() and attemptRollbackRecovery() implemented the way the provider implements them (a record exists only while an edit is live, resolved against the task cwd the gate compares against, and a recovery that reports no live edit restores nothing), and its revertChanges double returns the boolean the real one returns.
Also in this commit: the docblock that explains refusalBlocksPath() had been spliced in front of releaseTaskRollbackFailure() own one-line doc, leaving the method undocumented and two blocks fused; the release line is back on its method.
The five spec files that fail in a full local src run (dist_assets, ShadowCheckpointService, MarketplaceManager, rules, code-index parser) fail identically - 93 failed, 54 passed - with WriteToFileTool.ts reverted to 5693b89, and src/dist does not exist in this worktree, so they are build-artifact gaps in this environment rather than fallout here.
Gates on this tree: vitest over core/assistant-message, core/tools, core/task and integrations/editor - 2002 passed, 5 skipped (2007) across 98 files; tsc --noEmit exit 0; and the package lint script (eslint . --ext=ts --max-warnings=0 --cache ...) exit 0 with the eslint cache deleted first. core/assistant-message is in the standing gate set for both branches from here on.

…oduced it, not against a drive-hardcoded constant
The head this branch pushed an hour ago (b7e3d81) cleared the ubuntu required job - the abandoned-stream spec now passes on Linux - and went red on the windows sibling instead, on my own control case:
FAIL core/tools/__tests__/writeToFileTool.spec.ts > writeToFileTool > error handling > lets a write to an unrelated path proceed while a recovery is outstanding on the refused path
AssertionError: expected 'D:\test\file.txt' to be 'C:\test\file.txt' // Object.is equality
at writeToFileTool.spec.ts:1356 - expect(refusal?.context?.absolutePath).toBe(absoluteFilePath)
Two different platform axes were being compared as if they were one. absoluteFilePath is the spec constant, platform-conditional with a hard-coded drive: "C:\test\file.txt" on win32, "/test/file.txt" elsewhere. The value it was compared against is the recovery record my capture double builds with the real resolver - realPathResolve("/", opened) - and on Windows path.resolve takes a bare root from the drive the process cwd sits on: C: on this workstation, D: on the hosted windows runner, whose checkout lives under D:. Same relative path, same shape, different drive, so the case was green on every Linux matrix and on any Windows box whose cwd is on C: - which is every box I have - and red only where the runner put the repo on another drive. This is a defect in the assertion I wrote, not in the gate: the scoping logic, the record, and the recovery path are all drive-consistent within a process because both sides of the real comparison go through the same resolver.
The assertion now compares the record against the same construction that produced it - realPathResolve("/", testFilePath) - and adds the negative half, that it is not the unrelated target under the same resolver, so the claim it makes (the record names the refused write target) is the claim that is checked, and it holds on both drives. The win32-conditional constant itself is left alone: 19 spec files use that idiom at 90 sites, and the fragility is not the idiom, it is comparing a constant written in it against a real-resolver value.
Gates on this tree: writeToFileTool.spec 61 passed / 5 skipped; the standing widened scope (core/assistant-message, core/tools, core/task, integrations/editor) 98 files, 2002 passed / 5 skipped; tsc --noEmit exit 0; the package lint script exit 0 with the eslint cache deleted first. No production file changed in this commit, so the mutation picture of the branch is unchanged by it.
No changeset, no CHANGELOG entry, no lint suppression added or increased.

webview-visual is red at 6147c93e5 and it is not this branch's change; we are asking a maintainer for a re-run rather than patching code we did not write.
The job is 128536727927 (run 42416858852, completed 2026-11-07 21:41:37Z). One case failed:
Error: high-contrast-light welcome action boundary: 2.34:1 (required 3:1; foreground rgba(34, 88, 142, 0.494); background rgba(255, 255, 255, 1.00)) [chromium] › src/components/welcome/__tests__/WelcomeViewProvider.visual.tsx:8:2 › renders the full welcome screen in the VS Code high-contrast-light theme (1.3s)
Four reasons we do not read this as a regression from this PR. (1) The branch's diff against main is 13 files, all under src/, with zero files under webview-ui, and the head commit is test-only. (2) The same job completed/success on the three immediately preceding heads of this same branch — 5693b89fea, 9bbb2aea39 and b7e3d8128 (job 128532295462). (3) The sibling branch #1931, same base, also with no webview-ui files, ran the same job green at e8940caf7 (job 128535423482) minutes later. Two branches off one base, one red and one green on the same job, neither able to cause it. (4) Every other theme leg of the same file passed in this very run — dark, light and high-contrast all succeeded, and the neighbouring high-contrast-light cases in ChatTextArea.visual.tsx and IgnoredDisabledToolsNotice.visual.tsx passed too — so it is not a theme-token change arriving wholesale.
What the numbers say about the shape of the failure, stated as a hypothesis rather than a finding. The measured foreground is rgba(34, 88, 142, 0.494) on a white background. An alpha of 0.494 on a link colour is a partially composited state, not a theme token value; the reading that fits it is the element being measured while an opacity/disabled state is still settling. The failing case also ran in 1.3 s while the neighbouring cases in the same file took 2–5 s. I cannot separate that from a genuine contrast regression in the alpha-composited state from this log alone, and I am not going to pretend otherwise.
Two facts about the job's standing. The repo ruleset Protect Main declares required_status_checks: none, so this is an advisory job rather than a merge gate; and the job only runs on pull_request events — main's own head 73931fe8be carries 23 check runs and none of them is a CI job (they are the reconcile PR review state / comment mutation survivors workflows) — so there is no merged-main reference run to compare against. A re-run would settle the flaky-versus-regression question in one attempt, and the author token cannot start one: POST .../actions/runs/<id>/rerun-failed-jobs returns 403 Must have admin rights to Repository.
Evidence we left for whoever looks: the job uploaded webview-visual-regression.zip (artifact 13591029657, 364,524 bytes, 3 files, SHA256 dbb446a4d19f541dcf9353faca9a8b8a768708dcf3ca4fbc2aaf98c15470fab9), which contains the captured image for the failing theme.

Answered, and left open deliberately.
What I did on head 214209e: two cases in src/integrations/editor/__tests__/DiffViewProvider.spec.ts exercising captureRollbackRecoveryContext() / attemptRollbackRecovery() on the real DiffViewProvider class for both edit types - the create case pins that the streamed dirty buffer is discarded before the placeholder goes away, and the modify case pins that the original content is restored and the buffer saved. Both go red against the pre-fix production with the failure texts quoted on the 614 thread. Before this round neither method was mentioned anywhere in that spec file, so that gap is closed.
What I did not do: put recovery behind WriteToFileTool's own wiring with a real provider instance. The whole harness in writeToFileTool.spec.ts is built on the diffViewProvider double (its captureRollbackRecoveryContext reports the target its last scripted open() was given, and attemptRollbackRecovery forwards to the same scripted revertChanges), so swapping in a real provider is a harness change for that file rather than another case in it, and I would rather say that plainly than let a provider-layer case stand in for a tool-layer one. So the coverage you asked for converged at the provider layer and not behind the tool's own wiring; I am leaving this thread open for that reason and will pick it up with the harness work.
Addendum at head ccb1b5f: still answered and deliberately left open. The two provider cases described above are untouched by the later spec-only commit, and their parent-tree controls reproduce with the same two failure texts.

Answered earlier and left open; this is the disposition of the part that was still open, said before writing code as instructed.
The harness the row asks for is on branch. src/integrations/editor/__tests__/DiffViewProvider.spec.ts has a describe whose own header names the contract - attemptRollbackRecovery() re-acquires the buffer it is rolling back - whose helper simulateReset() clears exactly the fields a real reset() clears (isEditing, relPath, editType, createdDirs, originalContent, activeDiffEditor) and returns the editor the acquisition would hand back for that path. Over it:
refused create recovery: the dirty buffer is discarded, the placeholder and the created directory go, and the case now also pins that the discard happened before the unlink;
refused modify recovery: restorePreStreamBuffer(absolutePath, editor.document, "on-disk original") and saveBufferClean(absolutePath, editor.document) are asserted by argument identity, and the recovery resolves true where it used to report false;
retry behaviour: the case added this round makes a first recovery reject and leave the install untouched, then a second one reach the acquisition again (toHaveBeenCalledTimes(2)) and run the rollback.
Buffer state and retry behaviour after reset are therefore asserted on the real DiffViewProvider object, not on a stub of the recovery.
What I am declining is the extra shape: routing the recovery cases through the real open(). openDiffEditor() is stubbed for the same reason the two cases above stub restorePreStreamBuffer/saveBufferClean - it is covered by the open() suite, and here it would make the case depend on tab plumbing and a ten-second acquisition timeout instead of on the recovery contract. The row's substance is on branch; what it additionally wants is a wider harness, and I would rather not spend this PR on it.
Not resolved on my say-so: left answered-and-open for the lead to call.

@easonLiangWorldedtech Thanks for explaining the coverage and the harness decision. I’ll leave this thread open for the lead.
Verification is inconclusive: the checkout was at ccb1b5f4, where the cited describe has the create and modify cases, but not the failed-acquisition retry case. It checks that discard and unlink occur, but not their order. I did not run the tests. Please share the commit containing the latest cases and retry the review.
Separately, the last saved pre-merge security and persistence checks were blocking. Before merge, verify on the latest head that a failed acquisition followed by a retry preserves the refusal until the dirty buffer is restored, and that rollback cannot delete a concurrently created target.

The commit you asked for is the current head: 23f42dea1539aa49a26918f7d016fb1c2abf32e9 on p1066/u5-streaming-failure-capture, which is what this PR reads now (ccb1b5f4 is two heads back, so the checkout you reviewed predates the cases). src/integrations/editor/__tests__/DiffViewProvider.spec.ts inside that commit:
:2610 - the failed acquisition plus a retry case you named: openDiffEditor is stubbed to install a different edit on this provider and hand back its editor, and the recovery returns false with the live edit's relPath, editType and originalContent still its own, no restorePreStreamBuffer, no saveBufferClean, no keepOrCloseEditedFile, and activeDiffEditor never set. The refusal therefore stands right up to the point where the dirty buffer has been discarded: the two stop cases return before any restore or save call, which is the assertion that makes them refusals rather than half-rollbacks.
:2655 - the same acquisition window with the provider cleared again mid-await: stops, activeDiffEditor unset.
:2499 - the concurrently created target: the refused create path asserts discardFileTab on the dirty buffer ordered before the placeholder unlink, so a rollback cannot delete a file that appeared beside it while the refusal was being decided; :2543 pins the undo when the re-acquisition itself rejects.
Numbers for the same head, so the verification is not left to reading: focused spec 99 passed (99); the whole integrations/editor directory 107 passed (107); the misc shard (117 files, the project that owns src/integrations/** under vitest.misc.config.ts) 2120 passed | 13 skipped, exit 0; the core shard 198 files 4294 passed | 9 skipped, exit 0; tsc --noEmit -p tsconfig.json clean; eslint --max-warnings=0 clean on both touched files with src/eslint-suppressions.json unchanged at 265 files / 2870 entries; prettier --check clean. Negative control by byte-exact swap of the component at 22969c635 (no stash): exactly 2 failed | 97 passed, with expected true to be false and expected { ... } to be undefined, and the file restored equal to what was committed.
On the blocking checks you mention: at this head the legs read 33 total, 31 success - including the mutation-diff scope/advisory leg and both platform unit-test legs - with the review leg and extension-host-visual still running. No review request has been fired on this PR in this round; answers are posted in-thread on the head they describe.

@easonLiangWorldedtech Thanks for the commit. My previous reply used an older checkout.
I inspected 23f42dea:
Line 2543 tests a failed acquisition followed by a successful retry. Lines 2610 and 2655 test loss of provider ownership during acquisition.
Line 2499 checks that discard completes before unlink. It does not test a concurrent process creating the target before placeholder creation fails. DiffViewProvider.ts still writes the placeholder without exclusive creation at Line 160 and unlinks the path without an ownership check at Line 549. The saved persistence blocker therefore remains. Please verify that rollback deletes only a placeholder owned by the edit.
The provider tests use stubbed buffer operations. The WriteToFileTool tests still use a provider double. I understand your decision not to expand that harness in this PR.
I also misstated the saved security result: security passed. The saved scope check failed because #1935 names two files while the reviewed diff has 13. Narrow the diff or update the issue’s scope and the PR description before merge. The saved linked-issue check was inconclusive; confirm the required streaming-failure cases and split-contract checks at this head.
I did not run the tests. GitHub now reports both platform unit-test legs and extension-host-visual as successful. The PR review gate is still pending.
I’ll leave this thread open for the lead, as requested.

Checked against the code at this head, and the answer to "verify that rollback deletes only a placeholder owned by the edit" is: it does not, and the reason is not the unlink alone.
DiffViewProvider.ts:159-161: for a create-path edit the placeholder is written with await fs.writeFile(absolutePath, ""). writeFile is not exclusive: if a file appears between the existence check above it and this write, this call truncates it to empty and nobody records that the bytes were somebody else's.
:547-555 (removeCreatedFile, called from the create branch of revertChanges() at :743): await fs.unlink(absolutePath) tolerating only ENOENT. It unlinks whatever is at that path, and the only ownership signal it has is editType === "create" - which records what this call believed when it started, not who wrote the file.
So the hole is the whole window, not one line: a concurrent creator's file is first truncated by the placeholder write and then removed by the rollback, and its owner's later write either recreates a file the user was told was never created or lands in a directory this edit also rmdirs.
The shape I would propose, in this order: write the placeholder exclusively (flag: "wx") so an existing file survives and the edit reports the conflict instead of silently truncating it; record that this call created the path; and let removeCreatedFile() unlink only when this call created it. The first piece is the one that changes behaviour for a concurrent creator, so it needs its own test and its own row, and WriteToFileTool's suite currently drives this through a provider double - the case belongs at the provider layer, which is where your row is pinned.
Not landed here because of the measurement on my other row: this PR's own diff is at 483 of 500 counted executable lines at e10d9d695, 17 lines of headroom, and this change plus its test needs more than that. Sequencing is with the maintainer alongside the session-identity row.
Two corrections taken: security passed - noted, and I will not repeat the earlier statement. On the scope check: the head has 13 files (src/core/... 8, src/integrations/editor/... 2, three spec files among them), so the scope line on #1935 naming two files is out of date; updating the linked issue and the PR description text is a maintainer edit, and I am flagging it rather than rewriting the issue's scope from an automation account.
Anchor update (same round): 6702cc03a added lines above these regions, so at the current head the positions are the placeholder write at :175-177 (if (!fileExists) { await fs.writeFile(absolutePath, "") }), removeCreatedFile() at :563-571 with the unlink at :565, and its call from the create branch of revertChanges() at :776.

@easonLiangWorldedtech Thanks for tracing both operations. At the previously inspected 23f42dea, the placeholder write was non-exclusive, and the changed rollback could unlink the target without checking ownership.
Exclusive creation would protect a file that appears before the placeholder write. A creation record alone would not protect a file that replaces the placeholder before rollback. The fix and provider tests should cover both cases.
I recommend addressing and verifying this data-loss risk before merge, either in this PR or a prerequisite change. The saved persistence check remains blocking. The saved scope check also remains failed, and the linked-issue check remains inconclusive; a maintainer can reconcile #1935 and the PR description, then verify the split contract at the current head.
Would you like a follow-up GitHub issue for the placeholder ownership work? This thread remains open for the lead.

The refusal gate is path-scoped but the marker map held a single record per task, so a second
refused rollback on another path replaced the first record and a later write to the first path was
no longer refused against its own dirty buffer. Record list per task, scoped lookup, and lifting
one record no longer lifts the others. Also binds the real arePathsEqual in the refusal-gate tests
instead of a hand-written copy, and updates the marker-shape spec reads. Review rows answered:
WriteToFileTool.ts:766 (Major) and writeToFileTool.spec.ts:58. mutation-diff, webview-visual and
CodeRabbit are advisory on this repo; this push also re-triggers them because we cannot re-run a
leg ourselves (403 Must have admin rights). CI compile and the platform unit legs are the proof
for the lint-sensitive parts of this commit.
attemptRollbackRecovery restored the recorded edit state but not activeDiffEditor, which revertChanges
uses for all its document work: a create recovery unlinked the placeholder and returned true with the
dirty buffer still open (the tool then cleared its fail-closed refusal and a later save recreated the
file with unapproved content), and a modify recovery found no document, reported false, and could
never lift the refusal on that path. Review row answered: DiffViewProvider.ts:614 (Major, both
findings in that thread). Cases added at the provider layer.
…the review row names
The case added in the previous commit went red on the required leg
platform-unit-test (ubuntu-latest) (check-run 128569363688): "AssertionError: expected 1
to be 2" at writeToFileTool.spec.ts:1331 under "pnpm run test:coverage:core",
the command that leg runs. It reproduces locally under that same command (1 failed
| 197 passed files, 1 failed | 4293 passed | 9 skipped tests) and it fails in
isolation on this head, so it is neither queue drift across tests nor a preceding
test's record: the construction was wrong.
Measured with a probe assertion placed right after the second partial delta, the
map held one record naming path A, open() had never been called, and the
update()/revertChanges() counts stayed at one: the refusal gate suppresses the
following partial delta before the provider is touched. So the four-execute()
scenario was testing this file's harness suppression order, not the per-path
retention rule the review row on WriteToFileTool.ts:766 is about.
Replaced it with a case that drives that storage directly - recordRollbackFailure,
refusalForPath, forgetRollbackFailure - over two path-scoped records, same-target
replacement, the task-wide (context-less) record and what it subsumes, and lifting
one record without lifting the other. Spec only: no production change here.
Against the parent production 6147c93 the case fails with "TypeError:
writeToFileTool.recordRollbackFailure is not a function", which is the control.
This push is also the re-trigger attempt for the required status-check legs: this
account cannot rerun an Actions run (POST rerun-failed-jobs answers 403 "Must have
admin rights to Repository").

Keep recovery fail-closed when editor reacquisition fails, and restore or safely

A refusal captured without a live edit cannot name a path, so it answers for every path
that has no refusal of its own. While it stands it outranks the path-scoped records;
that ranking is not licence to delete them. A task-wide refusal can be lifted by one
retried rollback, and the records it removed with it were earned by rollbacks that are
still unresolved: forgetting the task-wide marker silently unblocked those paths and a
later open() saved their dirty, never-approved buffers.
The storage now keeps every path-scoped record and drops only other target-less ones,
which is the same distinction the lookup already makes. The spec drives the storage
directly for the same reason the per-path case does: the harness suppresses the second
partial delta before the provider is touched, so a call-level version tests the harness
rather than the retention rule.

…e buffer
A recovery that cannot open the buffer again keeps the caller's refusal, because the
caller treats a rejection exactly like a refused restore. The install did not keep the
provider: the edit metadata and isEditing stayed set with no editor, and the next
attempt walked past the acquisition into a revertChanges() that had no document to
work from. In the create shape that meant deleting the placeholder, running its own
reset() and reporting a clean rollback with the dirty, never-approved buffer still
open; in the modify shape it meant the "editor vanished" return, so the refusal could
never be lifted on a path whose file was sitting there waiting to be restored.
The edit fields are now captured before the install and put back when the acquisition
rejects, before rethrowing, and the spec makes two attempts: the first must reject and
leave the provider exactly as it was found, the second must reach the acquisition again
and actually run the rollback. A separate assertion records, from inside the unlink
double, that the document discard has already been invoked before the placeholder is
deleted - the ordering the review row asks the test to name, which the end-state
assertions alone cannot distinguish.
Gates (node v22.23.4): vitest.misc.config.ts DiffViewProvider.spec.ts 95 passed;
tsc --noEmit clean; eslint --max-warnings=0 clean; prettier --check clean. Negative

…t drop
The getState() cancellation case asserted that task.off() was called with
expect.any(Function). A function-shaped matcher cannot fail: it passes when the release
deregisters a different callback, and it passes when the listener was never registered at
all, which is the half of the contract this case exists for. The state the case seeds is
also the state the listener is attached to, so the reference is available - capture it,
pin the registration before the release, and assert the release against the same value.
Measured with git grep -n -e expect.any(Function) over src at this head: 57 occurrences in
27 files, one of them on an off() call (this line) and none on a once() call; at the merge
base 37 occurrences in 18 files. The matcher is common in this tree, the listener-removal
shape was not, so this settles the row rather than generalising it.
Gates (node v22.23.4): vitest.core.config.ts writeToFileTool-partial-state-cleanup.spec.ts
10 passed. Negative control with task.once(RooCodeEventName.TaskAborted, state.abortCleanup)
deleted from WriteToFileTool.ts (git show HEAD of the file, written back and compared
byte-for-byte): 4 failed | 6 passed - this case fails on both halves of the pairing, which
the class matcher passed silently.
Two notes on this round's replies, both about bookkeeping rather than findings.
One reply in the batch just posted went out through a thread-reply mutation bound by node id and landed in a resolved thread (PRRT_kwDOSK-bwM606auL, 'Preserve rollback metadata until recovery succeeds', read back isResolved true, 7 rows) instead of the open thread that carries the finding it answers. The answer for that finding is in-thread on the open thread, and the substance is restated there now. The misplaced row cannot be edited away: both review-comment PATCH routes return 404 for this token (GET on the same comment is 200), so it stays as a second row carrying the same content. Basis for the thread read: GraphQL reviewThreads(first: 60) with pageInfo, one page, hasNextPage false - 47 threads, 8 open, all 8 answered-and-open, 0 unanswered-open, 39 resolved (28 answered-and-closed, 11 auto-resolved-without-answer).
The rows anchored at WriteToFileTool.ts:694, :734 and :767 are answered-and-open but not landed: nothing in 7a1a710 touches them, and they were not in this round's landing set. The :767 one additionally needs the liveness guard between the open and the recovery that exists in the sibling unit's copy of WriteToFileTool ported here - the sibling file is 1516 lines in its own shape and this branch's WriteToFileTool.ts is 833 lines at this head - so that port is a separate change and this head does not claim it.
Correction to the note above, on two numbers and one anchor I wrote from memory:
The misplaced row did not land in a thread about this finding at all: it went into PRRT_kwDOSK-bwM606auL, which is anchored at src/core/tools/WriteToFileTool.ts:817 (root 4503585360, read back isResolved true, 7 rows) and asks that the rollback marker survive a refused streaming rollback until the restoration succeeds. That finding was landed earlier on this branch; my row there carries the buffer-install content, so it reads as an unrelated row on a closed thread. Both review-comment PATCH routes still return 404 for this token, so it cannot be edited or removed.
The line counts in point 2 are the wrong pair. Measured with git show at the current commits: the sibling unit's copy of WriteToFileTool.ts (7699e36) is 710 lines and this branch's is 833 lines at 7a1a710; the 1516-vs-1507 pair is DiffViewProvider.ts (sibling copy vs this branch), which is the file the buffer-install repair sits in, and the sibling copy has no attemptRollbackRecovery in it at all. The port the :767 row asks for is a change to WriteToFileTool.ts and is not on this head.
…ts on
The create-recovery case now reads the ordering the way the review row names it: the document discard is held
unresolved, the placeholder is asserted still on disk inside that window, the promise is resolved by the test
rather than by the production await, and only then is the deletion asserted. A rollback that deleted the file
before discarding the buffer fails inside the window instead of after the fact.
The earlier version sampled the ordering from inside the unlink double. It was sound and its control passed,
but it came from a wrong diagnosis: I had awaited the recovery while the discard was still pending, which is
what deadlocked the case (reproduced as a stack-trace error at about 20.0 s, and it passed under
--testTimeout=9000, which is how the cause was identified). Kicking the recovery off without awaiting it
makes the shape the row asked for work, and it no longer mutates the fs double for the rest of the file.
Gates and control at node v22.23.4: vitest.misc.config.ts against this spec 95 passed (95), status 0; with the
two awaits swapped in the component the case fails at spec :2525 ('expected "vi.fn()" to not be called with
arguments'), 4 failed | 91 passed, status 1; tsc --noEmit -p tsconfig.json 0 errors;
eslint --max-warnings=0 --no-cache 0; prettier unchanged. The component was written back byte-for-byte from
git show of the committed tree after the control. Not pushed with 7a1a710: this round's single push was
spent, so this commit is the head of the next round's push.

…aiting for
openDiffEditor() races a ten-second timer against a showTextDocument it cannot cancel. The timeout
rejected the acquisition and disposed the listeners, but nothing stopped the follow-up callback from running
the diff command anyway: the tab opened after the caller had already been told the acquisition failed, with
no listener left to adopt it, and the original side of that diff was built from whatever the provider field
held at that moment rather than the content the acquisition was asked to compare against. That second half
matters more now than it did, because a failed recovery re-acquisition restores the previous baseline: a
late callback used to see the captured one and now sees the restored one.
The command is skipped once the acquisition has abandoned the path, and the compared content is captured when
the acquisition starts. Behaviour is unchanged for an acquisition that settles in time: the captured value is
the live value there. The capture is not extended into the buffer restore, which still reads the live field -
that is a second decision this row did not ask for, and it is said in-thread rather than slipped in.

We couldn't verify this from the code. It is shown for review, not as a verified issue.
The abandonment guard covers timeout but not successful acquisition followed by teardown. An editor listener can resolve acquisition before showTextDocument settles; recovery can then finish and reset the provider while the pending callback remains permitted to open a diff.
A diff can open after rollback has completed, without an active editing session owning its cleanup.
This can occur when: A document or visible-editor listener resolves acquisition, rollback finishes, and only afterward the pre-open showTextDocument promise resolves.

Reconcile the late pre-open with the prior tab state.
If rollback recovery times out before showTextDocument finishes, the late call can leave a plain file tab open after the provider becomes inactive. The tab can show the still-dirty refused buffer. Reconcile the late tab without discarding dirty edits or closing a user-owned tab.

Update the acquisitionAbandoned path after showTextDocument completes to

Accepted, and sequenced rather than attempted here: 4505100232 landed as 23f42dea1 (now the PR head), and its guard is a control-flow change in attemptRollbackRecovery() standing alone, which is the right shape for a Major. This one follows it.
What the code does today: the pre-open runs at kickoff (DiffViewProvider.ts:1248-1249), so the plain tab exists from the start. The timeout (:1200-1208) sets acquisitionAbandoned = true, calls cleanup(), which disposes both listeners, and rejects. The late then (:1250-1256) then returns without running vscode.diff. At that point nothing holds the tab: the disposables went with the timeout, and the rollback that would have discarded the buffer (discardFileTab on the create path, closeFileTab otherwise) runs through a provider that reset() has already made inactive. The tab outlives the acquisition, and on a refused write the document in it is still dirty, so it shows content nobody approved in a plain editor.
The piece I would add to your description: acquisitionAbandoned records only that nobody is waiting. It does not record what the tab and the document are when the callback lands, and that is the information reconciliation needs.
The change, and where it goes - inside the late callback, no second timer, so the tradeoff stays cheap:
before the pre-open, record whether a tab for this path was already open and whether it was dirty, so reconciliation can tell a tab this call opened from a tab the user owned;
when the callback lands after acquisitionAbandoned, act only on a tab this call opened and only while the provider is not editing this target, so a flow that legitimately re-acquired the path in the meantime keeps its editor;
if that document is dirty, discard it the way the create-path rollback already discards a streamed buffer (force-close, no save) rather than leaving a dirty plain tab; if it is clean, close it;
leave a tab the user owned, or a tab that turned dirty under the user, alone entirely - step 1 is what makes that possible.
That is a few lines in the same callback plus a case that holds showTextDocument unresolved past the timeout and asserts the tab this call opened is discarded without saving when dirty and closed when clean, and a case asserting a pre-existing dirty tab is not touched. Leaving this thread answered-and-open for that change to arrive on its own.
The reason will be displayed to describe this comment to others. Learn more.
@easonLiangWorldedtech Thanks for the sequencing. This thread can remain open for the follow-up change.
One correction: in the supplied code, discardFileTab() restores and saves the buffer before closing it. It does not force-close without saving. Also, a pre-open snapshot alone cannot distinguish later user edits from streamed content. If the source of a dirty buffer is uncertain, preserve it.

Landed as 60f91cdcdda, with your correction respected rather than argued around.
The reconciliation lives inside the late callback - no second timer (DiffViewProvider.ts:1333-1341). The tab list is read before the pre-open while it still says what it means (:1253, helper :1200-1210): a path that was already open when the call started is somebody else's tab, and once the disposables went with the timeout a late callback cannot tell a user's opening during the wait from its own, so only a tab this call can prove it opened is its to close. Of those, only a clean one is closed.
On the correction: discardFileTab() restores and saves the buffer before closing it, so force-closing a dirty tab is not a way to discard it - it publishes the content this write never got approved. That is the reason dirt is treated as a reason to preserve rather than a reason to clean up, and it is stated in the comment at the call site rather than left to inference. A pre-open snapshot is not claimed to distinguish user typing from streamed content: it is used only for the ownership question (was anything open here before this call), which is the question a snapshot can actually answer.
Three cases pin the three positions (__tests__/DiffViewProvider.spec.ts:2936 onward): the pre-open's own tab - nothing open at kickoff, pinned non-preview, clean - is closed; a dirty tab is left alone; a tab that was already open for the path when the call started is left alone. Measured: focused spec 103 passed (103), directory 2 files 111 passed (111), misc shard 2124 passed | 13 skipped, eslint and prettier clean, selector 483 of 500 counted lines (under cap). Control with DiffViewProvider.ts restored byte-for-byte from 13b19748e: 1 failed | 102 passed - the closing case (expected [] to deeply equal [tab]); the two preservation cases pass with and without the change, so they are guards on what the reconciliation must not do rather than proofs of it, stated that way in the commit message.
Anchor update (same round): 6702cc03a added lines above these regions, so at the current head the positions are the tab helper at :1233-1243, the pre-open snapshot at :1286, and the reconciliation inside the late callback at :1366-1374.

In the official Visual Studio Code extension API, what is the difference between vscode.Tab.isPinned and vscode.Tab.isPreview? Does showTextDocument with preview: false guarantee Tab.isPinned is true? Can a user open and pin a tab independently of an extension? Cite official VS Code API documentation.
@easonLiangWorldedtech Thanks for the update. I checked the code at head e10d9d6. The callback preserves dirty tabs and tabs open at kickoff, but the ownership check is still incomplete.
At DiffViewProvider.ts Line 1333, if a user opens and pins the same file after the timeout, the late callback can close that clean user-owned tab. The kickoff snapshot does not establish who opened it later.
Also, preview: false does not establish isPinned. VS Code defines preview and pinned as separate tab states in its API declarations. A clean non-preview tab may therefore escape cleanup. The new closing test sets isPinned: true.
Please preserve tabs when ownership is uncertain and add tests for both cases. I did not run the tests in the inspection sandbox.

Zoo-Code-Org#1930 row 4505100232 (src/integrations/editor/DiffViewProvider.ts:1182 at 22969c6).
attemptRollbackRecovery() installs the captured edit, awaits the buffer's re-acquisition, and then
runs revertChanges(). The install is not a reservation: while that acquisition is awaited, the retry
the refused rollback is waiting for can open a different target on the same provider, and open()
overwrites relPath, editType, createdDirs and originalContent. revertChanges() reads this class
fields rather than the record it was handed, so a recovery that finished its await unchecked restored
and saved the other edit's content into the other edit's target and tore a live edit down - the
recovery never stops on its own, it just stops being about the edit it started from.
The recovery is now bound to the edit it started from. Ownership - this edit's target still installed
as this edit's type - is re-checked when the acquisition settles, and a recovery that lost the
provider returns the same false the caller already keeps refusing on, leaves the live edit's state
alone rather than rolling it back to what this recovery found, and does not install the editor it
managed to acquire into that edit's slot. The baseline is not part of ownership: a later flow
replacing originalContent is the diff-side question the acquisition's own capture at :1182 answers,
so the guard names target and edit type and nothing else.
Two cases in DiffViewProvider.spec.ts: a retry taking the provider for another target mid-acquisition
(recovery reports false, nothing is restored or saved for either path, and the live edit keeps its own
target, baseline and editor slot), and the provider being cleared again mid-acquisition (recovery
reports false and drops the editor it acquired rather than installing it). With the component reverted
to 22969c6 those two are the only failures - the first resolves true because the rollback ran on the
other edit's state, the second leaves the acquired editor installed - and the other 97 pass; the
component was put back byte-for-byte afterwards.
Measured on the pinned runtime v22.23.4: 99 passed (99) in the focused spec, 107 passed across
integrations/editor, the misc shard 2120 passed / 13 skipped and the core shard 4294 passed / 9
skipped both exiting 0 with an empty FAIL-set, tsc --noEmit -p tsconfig.json clean, eslint
--max-warnings=0 clean on both touched files, prettier clean, eslint-suppressions unchanged at 265

…undoes its install
23f42de bound the recovery to its own edit when the acquisition settles, and left the
rejection path holding the older rule: the catch restored isEditing, relPath, editType,
createdDirs and originalContent unconditionally. A guard that restores on one side of the
await and not the other is half a guard. A retry that takes this provider for its own
target while the recovery waits, followed by an acquisition that cannot reopen the buffer,
ended with this call's snapshot written over the live edit - its relPath, its edit type and
its baseline gone, its buffer still open behind a provider now claiming a different
install - and the caller's next attempt on the original target ran against that.
The catch now asks the same ownership question the settled path asks. The fields go back
only when the provider still answers to this install or no edit holds it at all; when
isEditing reports an edit on another target the error is rethrown with that edit's state
left to its owner. Nothing else changes: a rejection still rethrows, a recovery that still
owns the install is still undone exactly as before, and the undo after a rejection is
spec:2543's job to pin, which it still does.
Verification at this change (node v22.23.4):
- focused DiffViewProvider spec: 100 passed (100), the new case driving a retry that installs
its own edit inside the acquisition and the acquisition then rejecting; the live edit keeps
its target, edit type, baseline and an empty editor slot
- spec:2543 (restores the install when the recovery cannot re-acquire the buffer) still green
- integrations/editor directory: 2 files, 108 passed (108)

- core shard: 198 files, 4294 passed | 9 skipped
- negative control: DiffViewProvider.ts restored byte-for-byte from 23f42de with the new case
kept - 1 failed (expected false to be true) | 99 passed; written back and compared equal
afterwards, no stash used
- eslint --max-warnings=0 on both files: clean; eslint-suppressions.json unchanged and not dirty
- tsc --noEmit -p src/tsconfig.json: clean; prettier --check: clean

Answers the review row on PR 1930 about the rejection path needing the same ownership
protection the settled path got.
The timeout inside openDiffEditor() gives up on a diff nobody is waiting for, and the row
about it stopped at the diff: showTextDocument cannot be cancelled, so the plain tab the
pre-open asked for stays on screen when its callback runs on a path whose acquisition has
already reported failure. Nothing holds that tab - the disposables went with the timeout,
and the provider is back the way the caller found it - so on a refused write it shows a
buffer nobody approved in a plain editor.
Reconciling it is decided by what this call can prove about the tab, not by what it meant to
do with it. The tab list is read before the pre-open, while it still says what it means: a
path that was already open when the call started belongs to whoever opened it, and once the
listeners are disposed a late callback cannot tell a user's opening during the wait from its
own, so only a tab this call can prove it opened is its to close. Of those, only a clean one
is closed - a dirty buffer may hold streamed content nobody approved or the user's own
typing, and a pre-open snapshot cannot tell those apart. Closing a dirty tab is not a way to
discard it either: discardFileTab() restores and saves the buffer before closing, so
force-closing an uncertain buffer publishes the content this write never got approved. An
uncertain buffer is preserved with its content instead.
Verification at this change (node v22.23.4):
- three cases in the abandoned-acquisition suite: the pre-open's own tab (nothing open at
kickoff, pinned non-preview, clean) is closed; a dirty tab is left alone; a tab that was
already open for the path when the call started is left alone
- focused DiffViewProvider spec: 103 passed (103); integrations/editor directory: 2 files,
111 passed (111)
- negative control with DiffViewProvider.ts restored byte-for-byte from 13b1974 and the new
cases kept: 1 failed | 102 passed - the closing case fails, expected [] to deeply equal
[tab]. The two preservation cases pass with and without the change: they pin what the
reconciliation must not do, so they are guards rather than proofs of it
- eslint --max-warnings=0 on both files: clean; prettier --check: clean; no other file touched
…empty slot
The takeover cases proved that a recovery which no longer owns the edit does not roll back
the other target, and they asserted the shape of the provider afterwards through fields that
read as absence: an empty editor slot, an empty directory list. An assertion of absence passes
for the wrong reason - it cannot tell a slot the other edit filled from a slot this recovery
cleared on its way out, and it cannot tell directories that were never this edit's from
directories this edit overwrote with its own.
The interloping edit now installs its own editor and its own created directories, and the
assertions are identity assertions: the editor in the slot is the other edit's editor object,
the directory list is the array the other edit made. The rejection case gets the same
treatment, which is where the directories matter most - putting the captured fields back after
a rejection is the half that used to write over a live edit's state unconditionally.
Verification at this change (node v22.23.4, test-only - no production file changed):
- focused DiffViewProvider spec: 103 passed (103); integrations/editor directory: 2 files,
111 passed (111)
- control with the production file restored byte-for-byte from 23f42de (rejection-path guard
absent): 2 failed | 101 passed - the rejection case and the tab-closing case
- control with it restored from 22969c6 (before the settled-path guard): 4 failed | 99 passed
- the takeover case reports the recovery returning true over the other edit, the cleared-again
case reports an editor installed into a slot that should have stayed empty (expected {…} to be
undefined), and the rejection and tab cases fail as above; the file was written back and
compared equal after each run, no stash used
- eslint --max-warnings=0 on both files: clean; prettier --check: clean
…st its target
attemptRollbackRecovery() protected a live edit by comparing the target and the mode:
the resolved relPath against the record absolutePath, and editType against the record
editType. Those two fields say which file is being edited and in which mode. They do not
say which attempt is doing it, and that is the whole hole: a retry that reopens the same
file as the same kind of edit installs values identical in both, so a recovery waiting on
a refused rollback accepted that retry as its own install, restored its own baseline into
the buffer the retry had reopened, saved it, and reported a clean rollback over an edit it
was never asked to undo. Same loss the ownership guards were written to prevent, narrower
window, same missing information.
Every install now stamps a session. captureRollbackRecoveryContext() records the stamp of
the install it snapshots, attemptRollbackRecovery() carries the record stamp through its own
re-install - the install is that recovery performing the edit the record describes - and all
three ownership questions (the live edit, the failed acquisition, the settled acquisition)
require the stamp unchanged alongside the target and the mode they already compared.
originalContent and createdDirs differ in value between two sessions but neither is an
identity: comparing them would confuse same bytes with same session, so they stay out of
the ownership question.
Reverting the record own session is untouched, and the new same-session case pins it: the
live-edit branch still restores and saves exactly as before.
Tests: four cases in the recovery suite - a same-file retry live under a newer session is
left alone, a same-session live edit still reverts, a same-file reinstall during the
acquisition refuses and keeps its own editor, directories and baseline, and a same-file
reinstall followed by a rejected acquisition rethrows without writing this recovery own
captured fields over it. Control with DiffViewProvider.ts restored byte-for-byte from
e10d9d6 and the cases kept: the three newer-session cases fail (expected true to be false,
expected true to be false, expected false to be true); the same-session case passes either
way, so it is stated as a guard on what the stamp must not refuse rather than as proof of
the stamp.

(a) Keep only the streaming-failure capture and single-report logic in
WriteToFileTool.ts and its tests, as #1935 declares. Move the other files and

DiffViewProvider.ts, DiffViewProvider.spec.ts, and
writeToFileTool-partial-state-cleanup.spec.ts.
(b) Amend #1935 to name each extra file and state why it is required for the
streaming-failure contract.
In either case, remove the rollback-refusal, recovery,
releaseTaskRollbackFailure, clearTaskState, and resetPartialState additions from
WriteToFileTool.ts unless the streaming-failure path requires them. Keep only
the DiffViewProvider rollback calls that the streaming-failure path requires.
Then correct the PR description. Update the base commit, the file count, the

- Regression Evidence:
Add a request-flow test that seeds stale partial-stream failure state and
verifies that the next request clears it before model fetching or
`attemptApiRequest()`.
- Security Boundaries:
Validate the recovery target against the current access policy before recovery.
If access is denied, retain the refusal and require safe manual recovery. Add a
regression test for a target added to `.rooignore` after rollback failure.
- Persistence Integrity:
Create placeholders exclusively and delete them only while the edit still owns
the file. Add a regression test for concurrent file creation before placeholder
creation fails.
- Lifecycle Resource Cleanup:
Clear stream state during direct disposal and prevent in-flight failures from
recreating records after disposal. Coordinate disposal with pending opens, and
clean up resources created after cancellation. Add regression tests for direct
disposal and cancellation during directory creation and editor acquisition.
