Part of the file-write-safety series (#1375) — B1: per-write checkpoints, task-start baseline, and the perWriteCheckpoints setting.
What
Per-write checkpoints — write_to_file, edit_file, and apply_patch now record one suppressed checkpoint after each successful write. apply_patch records exactly one checkpoint for the whole patch (not per file).
Task-start baseline — initiateTaskLoop records a one-time task-start baseline checkpoint, guarded by taskStartBaselineDone (the loop runs on both start and resume, so the guard makes the baseline fire exactly once per task instance).
perWriteCheckpoints setting (default-on) —
defined in packages/types/src/global-settings.ts (DEFAULT_PER_WRITE_CHECKPOINTS + z.boolean().optional()) and included in ExtensionState (vscode-extension-host.ts),
exposed as a checkbox in CheckpointSettings and wired through SettingsView's cachedState round-trip (initial value + handleSubmit payload),
persisted through the generic ContextProxy path (same shape as enableCheckpoints — no special webviewMessageHandler handling),
surfaced by ClineProvider.getState() and getStateToPostToWebview() with the shared default,
default-on in the webview's createInitialExtensionState (mirrors enableCheckpoints).
Live, default-on semantics — hooks read the current state and skip only when the setting is explicitly false.
Tests
Task.spec.ts — task-start baseline: exactly one suppressed save on the first initiateTaskLoop run (guard verified across two loop invocations); no save when the setting is off; the baseline save is awaited before the first request (a held-promise regression test proves the request loop does not start first).
writeToFileTool / editFileTool / applyPatchTool specs — one suppressed save per successful write; none when the setting is disabled; none on the failure path.
ClineProvider.spec.ts — saved-value and default round-trips for both getState and getStateToPostToWebview (true / false / unset).
New CheckpointSettings.spec.tsx — default-on checkbox binding, saved-false and saved-true bindings (the explicit persisted value, not only the default-on fallback), and toggle caching through setCachedStateField.
i18n perWrite key (label + description) added to all 18 locales.

kind: branch - feat/per-write-checkpoints-b1, refreshed onto current main by merge b01a334e4 (upstream/main 9c069e94ef) so this branch carries nothing main already has. No unit content changed in the refresh.
Budget (own delta at this head)
36 files, +914 / -19 vs main 9c069e94ef (tree-to-tree). Authored code is well under the series cap; ~34 lines are CI-mandated i18n parity across 18 locales.
Expected: all green; src/eslint-suppressions.json unchanged.
Verification at this head (a0d292efe)
CI: 19/19 checks green at head (incl. check-translations, platform-unit-test ubuntu+windows, compile with the new format:check step, knip, e2e-mock, Build test VSIX).
Local: ESLint --max-warnings=0 clean on all 10 touched src files; tsc --noEmit clean; src vitest 469 passed / 5 skipped / 0 failed (5 files, at the merge head; re-run after the follow-up: Task.spec 177/177 green); webview-ui vitest 28 passed (2 files); translations check clean; model-check 9/10 locally (the 10th script fails only on the local Node 25 + tsx ESM-resolution defect - reproduces on an untouched main checkout; green in CI).
Prettier: clean on every file in the unit diff. src/eslint-suppressions.json: no added or increased entries.
Updated this round (refresh onto main 9c069e94ef)
b01a334e4 - merge upstream/main. Two conflicts, both test files, resolved keeping both sides: ClineProvider.spec.ts keeps this PR's five perWriteCheckpoints state tests and main's two blanket-auto-deny tests; ExtensionStateContext.spec.tsx takes main's shared DEFAULT_WRITE_DELAY_MS placeholder shape (writeDelayMs: 0) and keeps this PR's perWriteCheckpoints key. No production-code conflicts.
Update (CodeRabbit-sync from trial #1413): head b98f15903 — createInitialExtensionState exported + focused test asserting the pre-hydration perWriteCheckpoints default (trial addendum 4fc14c4). Review context: trial PR #1413.
Review-gate re-trigger (2026-08-30): empty commit abc10b4 (no code change) re-runs CI and CodeRabbit current-head review under the org new PR review gate; the code head remains b98f159.
Visual regression note (2026-08-30): The advisory extension-host-visual check (chat-dark sidebar snapshot) fails on this branch with a deterministic 973-pixel delta localized to the completion-result area. Root cause: the B1 task-start baseline checkpoint (per-write checkpoints default on, allowEmpty) adds a suppressed checkpoint_saved row to the message history, which getCompletionCheckpoint() picks up, so SeeNewChangesButtons render below the completion row; the suite baseline (recorded pre-FWS in #1426) has no such buttons. Every other pixel is identical, and the delta is pixel-identical across all branches containing B1 (1404/1406/1410/1411/1412/1413), while non-B1 branches are green. This is a new-expected-behavior vs stale-baseline mismatch rather than a rendering regression. Flagged for maintainers: either update the electron-chat-dark-sidebar.png baseline on the B1 branch or adjust the scene/component behavior. Advisory check only — it does not block the required gates.
c07bea302 - review follow-up: task-start baseline is now skipped on history resumes - initiateTaskLoop gates the baseline on _isHistoryTask so a resumed task no longer records a second baseline (design: exactly once per task; the resumed shadow repo already carries the original baseline). Regression test added; negative control verified (reverting the guard reddens the new test). Ported to the sibling units (PR 1406 ba5908ba2, PR 1411 c3bef96f7, PR 1410 8386e3739, PR 1412 d125039af).
93e3640e4 - review follow-up (ported from PR 1410): baseline skipped on already-aborted tasks (regression test added; Task.spec 178/178), and WriteToFileTool/EditFileTool now read perWriteCheckpoints through the shared DEFAULT_PER_WRITE_CHECKPOINTS constant. tsc clean, eslint clean, Prettier clean.
faf3f9d33 - review follow-up (ported from PR 1410): the baseline block rechecks this.abort after the getState() lookup, so cancellation during the deferred state lookup skips the baseline save; regression test added (Task.spec 179/179, negative control verified). tsc clean, eslint clean, Prettier clean.
fb576709d - review follow-up (Persistence Integrity row - fixed in code, not argued): EditFileTool and ApplyPatchTool fired checkpointSave without awaiting it, so the diff-state reset and the queued-work handoff could race an unfinished checkpoint commit (and shutdown could lose it). Both now await the save, matching WriteToFileTool and the semantics the later units of this series settled on. Held-promise regression tests pin the ordering on both paths (negative controls verified: reverting to fire-and-forget reddens both). Tool specs 92 passed / 5 skipped, tsc clean, eslint clean, Prettier clean.
cae09fbd5 - review follow-up (Regression Evidence row): added the requested multi-file apply_patch test asserting exactly one checkpointSave for the complete patch (B1's one-checkpoint-per-patch contract). The row's SettingsView half is owned by the unit that ships the control (see PR 1411's dispositions); the base unit's provider/round-trip tests already cover the setting here. Ported context: the same await fix landed on PR 1406 (4c0bc15bc), the only other unit still carrying the unawaited EditFileTool call.
c6a23c511 - review follow-up (queued-message handoff, raised on PR 1404): the per-file handlers processed queued messages before execute awaited the whole-patch checkpoint, so a queued turn (and its own checkpointed writes) could start mid-commit. The handoff is now deferred to after the checkpoint attempt - one handoff per successful patch, independent of the checkpoint setting or outcome - matching the deferral the journal-carrying unit already shipped. Tests: held-promise deferral (negative control verified), handoff with checkpoints disabled, no handoff on failure. Tool specs green, tsc/eslint/Prettier clean.
484d5cf0e - review follow-up (Lifecycle Resource Cleanup row): processQueuedMessages dequeued unconditionally, and since tools now await the checkpoint before handing off, a cancellation landing during that await consumed the queued message and dropped it (submitUserMessage rejects once abort is set). The dequeue is now skipped while the task is aborted - guarding every handoff site at the lowest layer - with a regression test (negative control verified: the unconditional dequeue reddens it). Task spec 180 passed.
484d5cf0e - review follow-up (Regression Evidence row, settings half): SettingsView save round-trip tests assert the durable payload - default-on perWriteCheckpoints: true when the setting is unset, and an explicit perWriteCheckpoints: false after unchecking and clicking Save (webview-ui per AGENTS.md is the layer for React forms/save wiring; the toggle binding itself was already covered by CheckpointSettings.spec). Webview settings specs 34 passed, webview tsc clean.
Review dispositions (registered once)
Lifecycle Resource Cleanup (task-start baseline cancellation row): by design, bounded. initiateTaskLoop already checks abort before the baseline and rechecks after the state lookup, so a canceled task performs no new checkpoint work; the only remaining window is a cancellation landing while the single awaited baseline commit is in flight, which costs at most one bounded shadow-git commit that dispose then reaps. Threading an AbortSignal through getCheckpointService/checkpointSave (the suggested fix) is a checkpoint-service-wide redesign beyond this unit's fidelity contract - the same posture as the registered journal-batch decision on the journal unit.
Functional Correctness (partial-failure handoff thread): argued - see the thread reply. Requiring full-patch success would strand queued input on any partial failure, changing upstream behavior (handlers handed off after each successful write); the deferral keeps the upstream condition (a write happened) and moves only the timing after the checkpoint attempt. Pinned by two tests at 0310c99.
Persistence Integrity (move/checkpoint row, PR 1404 walkthrough): superseded and fixed at a0d292efe. The earlier by-design disposition (checkpoint the half-finished move) no longer reflects the shipped behavior: when the source unlink fails after the destination write, the handler now restores the pre-move state (removes the destination with fs.rm(..., { force: true }), reports the failure through task.say + a tool error, resets the diff view, and returns failure), so the checkpoint never records a half-finished move and the operation fails explicitly. Pinned by the test "restores the pre-move state and fails the operation when source deletion fails"; negative control: removing the restore made exactly that test red.
Regression Evidence (Playwright fixture half): by placement. AGENTS.md routes React rendering/forms/save wiring to webview-ui tests, which cover the control (CheckpointSettings.spec) and the save payload (SettingsView.spec, added at 484d5cf); apps/vscode-e2e is reserved for real-host behavior, and the settings toggle has no extension-host-specific behavior to snapshot. The webview-visual CI job already pixel-covers the settings surface.
0310c997e - review follow-up (partial-failure handoff thread): argued, not changed - upstream handed off in each handler after every successful write, so partial failures and access-check early returns drained the queue; the deferral keeps that condition and moves only the timing. Two fidelity tests pin it (partial failure still hands off; access-check early return still hands off). Execute specs 31 passed.
a83a105ff - Stryker-directive sweep: removed the seven // Stryker disable next-line comments carried in Task.ts's streaming/telemetry region (per the updated AGENTS.md: directives are removed when their file is touched; comment-only, no behavior delta). Equivalence notes for any mutants that now survive there (mutation-diff is advisory): event.name is always a string on those paths; streamingKey can never be undefined there; the getStreamingKey fallback region is unreachable defensive code. eslint/tsc/Prettier clean, Task spec 180 passed.
Test-only commit: the early-return handoff test now asserts both that no whole-patch checkpoint is saved and that the handoff occurs exactly once, per the review thread (negative control: duplicating the finally-block handoff reddens exactly that test). eslint/tsc/Prettier clean.
Refreshed onto upstream/main (tool-call state scoping): ToolCallbacks gained a required toolCallId; the execute spec's callback fixtures pass it (test-tool-call-id, matching main's own spec convention). No production-code change. Full-src eslint 0, tsc 0, execute spec 25 passed, Task spec 191 passed.
Test-only commit: the in-place update test previously asserted nothing; it now asserts no error handling ran and exactly one whole-patch checkpoint with the standard arguments (negative control: skipping the default-on save reddens it). eslint/tsc/Prettier clean, spec 25 passed.
The flagged move handling is unchanged upstream code, not a regression introduced here:
The write-destination / best-effort fs.unlink(source) / return-success sequence (ApplyPatchTool.ts lines 480-484) is attributed by git blame to upstream commit 0db51d8bf (2026-03-03); this PR's diff against upstream/main contains no hunk touching the unlink region.
The same window exists on main today: main checkpoints after a successful tool result (src/core/task/Task.ts:2400void this.checkpointSave(false, true)), so a locked source already produces a checkpoint containing both source and destination before this PR.
The partial-write retention the row cites (retain earlier successful writes when a later handler returns false, no rollback) is likewise main's behavior; this PR only changes checkpoint granularity and handoff, both covered by their own tests.
Making moves transactional (destination removal, source restore, whole-patch rollback, delayed handoff) changes upstream move semantics repo-wide and is out of scope for the checkpoint-granularity unit; registered as a follow-up candidate for maintainers.
Persistence Integrity row (04:13 walkthrough) - implemented as demanded
Move handler now returns false when the source deletion fails: the
destination write is removed (pre-move state restored; a failed
restore is logged), the failure is reported through the tool error
result, and the whole-patch checkpoint is no longer recorded over a
duplicated file.
Multi-file patches now follow one transactional policy: successful
operations retain their writes, the failed operation reports itself,
and before the queued-message handoff an explicit partial-state
checkpoint persists the retained writes (the access-check failure
path routes through the same branch). The whole-patch checkpoint is
still recorded only after every filesystem operation succeeded.
Negative controls: restoring the swallow made exactly the move test
red; disabling the partial-state branch made exactly the three tests
asserting it red; both restored byte-exact.
Upstream refresh (this push)
upstream/main advanced (threshold commit for the series), leaving the branch conflicting with main on the regenerated e2e screenshot asset only; merged upstream/main into the unit branch and resolved apps/vscode-e2e/src/visual/__screenshots__/electron-chat-dark-sidebar.png to main's version. The PR diff remains unit-only; no source hunks were altered by the merge.
CodeRabbit inline thread (Minor, quick win): when a later file operation rejects outright after earlier handlers succeeded, the outer catch skipped both checkpoint branches while the finally block still handed off queued messages — the queued turn could start with the retained writes carried forward without rollback metadata.
Fix (this push): the catch now persists an explicit partial-state checkpoint before handleError and the handoff when handoffPending is true, with the same default-on semantics as the success/partial branches (skip only when perWriteCheckpoints is explicitly false; save failures stay suppressed so the original error still reports). Test: persists a partial-state checkpoint when a later handler rejects outright after an earlier write. Negative control: removing the catch-block checkpoint turns exactly that test red (1 red, the new behavior).
Review follow-ups (row 10)
The CR wave on the previous head is fixed in a0d292efe:
T2 (task-start baseline): the baseline block is wrapped in try/catch - a rejected provider-state lookup or a checkpoint error now logs [Task] task-start checkpoint baseline failed: and task startup continues; the baseline is best-effort and runs before TaskStarted, so it must not abort the loop (startTask rethrows non-abort errors). Test: "continues task startup when the baseline checkpoint fails" (negative control exactly-1-red).
Merged upstream main f707b6dfe (clean merge; gates and affected suites re-run green). Upstream enabled knip --production, which fails test-only production exports. Response on this head:
No knip findings in this unit's diff; refresh only.
Updated this round (row-13 walkthrough fixes at 8865c4ad1)
Move rollback restores a clobbered destination (fresh walkthrough claim, Persistence Integrity row): the update-with-move path now captures the destination's prior content before writing the moved file. If the source unlink fails, the destination is restored to that captured content instead of being removed, so a failed move no longer destroys a pre-existing file. When the destination cannot be inspected at all (a non-ENOENT read failure), the move is refused before any write rather than treating the file as absent. Negative controls: reverting the restore branch reddens exactly the restore test; reverting the inspect guard reddens exactly the refusal test.
perWrite setting wording (fresh walkthrough claim on the settings thread): the description now states that a multi-file patch produces a single checkpoint, matching the shipped behavior (one checkpoint per patch, one journal entry per write). Updated in all 18 locales; labels and keys unchanged.
Row-14 wave (round 49)
Task-start baseline hardening (ported byte-identical to all five units): a rejected settings read now falls back to default-on instead of aborting task startup, and the baseline-done flag is set only after an explicit disable/abort or a save that actually committed - a baseline that no-ops behind a failed or timed-out service initialization is retried on a later loop entry. Regression tests: "starts the task default-on when the baseline settings read rejects" and "retries the baseline on a later loop entry when a save produced no commit"; negative controls each exactly-1-red.
getCheckpointService never stores an uninitialized service: a failed Git check or initShadowGit now resolves to no service (and clears the initializing flag) instead of storing a dead handle. New regression spec getCheckpointService.spec.ts (init-failure and git-not-installed must not store; success path stores); negative control exactly-2-red.
The Persistence Integrity walkthrough row at 03:05Z describes the move path as it existed BEFORE the destination-preservation fix landed on this branch; the fix is present at the reviewed head 911ae24:
"It does not reject or preserve an existing destination" - false at this head: ApplyPatchTool.ts:491-510 captures the destination content before either supported write path and REFUSES the move before writing when the destination exists but cannot be inspected (non-ENOENT read error).
"the new rollback calls fs.rm(moveAbsolutePath, { force: true }) ... which deletes the prior destination contents" - false at this head: the unlink-failure rollback (:528-545) restores the captured destination content via fs.writeFile and calls fs.rm only when the destination was confirmed ABSENT before the write (destinationBackup === undefined), so prior destination data is never deleted by the rollback.
"the code only logs the error and returns failure, leaving both source and destination" - accurate only for the double-failure case (source unlink AND destination restore both fail - an unrecoverable filesystem state); the log-and-fail behavior there is the tested contract.
Test coverage: applyPatchTool.execute.spec.ts "restores the pre-move state and fails the operation when source deletion fails" (:150), "restores an existing destination when source deletion fails after the move clobbered it" (:194 - the exact clobber case the row claims is unhandled), "refuses the move before writing when the destination cannot be inspected" (:236). The row's cited :149-180 is the absent-destination test; the clobber-restore test at :194 is not acknowledged.
Verdict: STALE (CR re-anchored pre-fix hunks). No code change; registration recorded here.

Summary
Summary by CodeRabbit
New Features
Automatically save a checkpoint after successful file writes. A multi-file patch creates one checkpoint for the entire patch.
Create a baseline checkpoint when a task starts.
Per-write checkpoints are enabled by default and can be turned off in Checkpoint settings.
Setting labels and descriptions are available across supported languages.
Bug Fixes
Keep queued messages untouched when a task has been aborted.
Restore checkpoint availability after initialization failures and serialize checkpoint saves.
Protect existing files during updates and moves, with improved recovery when an operation fails.
The change adds a default-enabled per-write checkpoint setting and exposes it through provider state and the settings UI. Eligible tasks attempt a baseline checkpoint at startup. Successful writes and patches attempt checkpoints. Checkpoint saves are serialized per task, and editor direct saves use staged writes before rename.

Explanation
The title "feat(checkpoints): per-write checkpoints, task-start baseline, and perWriteCheckpoints setting (B1, #1375)" matches the main changes in the changeset. The diff adds per-write checkpoints for write_to_file, edit_file, and apply_patch, a task-start baseline checkpoint in Task.ts, and the perWriteCheckpoints setting across types, provider state, settings UI, and i18n. The title is accurate and specific. It is long at 106 characters, and the "(B1, #1375)" series tag adds noise, but neither makes it unrelated or vague.
Full details: Description check
Explanation
The description follows the repository template in structure. It includes a tracking issue reference and a Description-like "What" section that explains the per-write checkpoints, task-start baseline, and setting. It has a Test Procedure section with commands and a Pre-Submission Checklist. However, the template's required "Closes: #<issue>" line under "Related GitHub Issue" is absent. The author links a tracking issue (#1397) and a parent series (#1375) instead, and the description does not use the template's "Closes:" format. The description also contains a large amount of non-template material: review dispositions, CI and visual-snapshot notes, a "Content source of record" section, budget numbers, and multiple dated follow-up logs. These make the description hard to scan and conflict with the template's instruction to keep the PR focused. Several sections the template expects are weak or missing. The Visual Snapshots and Videos sections are not filled in, even though the PR touches UI settings and the description reports a visual-regression mismatch. The Documentation Updates checkbox is not ticked. The Get in Touch section is empty. The description also uses terms like "B1" and "row-13 walkthrough" that a reader outside the review thread cannot follow. Because the core content is present, the check passes, but the author should add the issue link in the required format, trim the review-log material, and state the visual-snapshot and documentation status.
Resolution
Consider these changes before merge: (1) Replace the tracking-issue line with the template's "Closes: #<issue number>" line that links an approved issue. (2) Move the review dispositions, dated follow-up logs, CI status, and budget numbers out of the description, or condense them into a short summary. (3) Fill in the Visual Snapshots section. The description reports an extension-host-visual mismatch on this branch, so state whether a snapshot baseline was updated or why no snapshot applies. (4) Tick or explain the Documentation Updates checkbox. (5) Add a Discord username under Get in Touch if the template requires it.
Full details: Security Boundaries
Explanation
The changed staging paths bypass file controls and can expose secrets. atomicWrite.ts:33-37 and ApplyPatchTool.ts:555-566 use predictable names and follow existing symlinks. A local attacker who can write to the destination directory can plant a staging symlink to redirect an approved write to another file. Also, ApplyPatchTool.ts:643-645 writes recovery content with default permissions. If source deletion and destination rollback both fail, a recovery copy containing secrets can become readable to other users under a typical umask.
Resolution
Create unpredictable staging files with exclusive, no-follow creation in both write paths. Create recovery files with exclusive owner-only permissions, and test symlink attacks and recovery-copy permissions.
Full details: Persistence Integrity
Explanation
The prior pushToolWriteResult failure is addressed by the mutation tracker and regression test. Other changed paths can still lose state: - ApplyPatchTool.ts:314-324,680-689: saveDirectly and saveChanges can write a file and then reject during editor or diagnostics work. The mutation flag is not set until they return, so the catch skips the partial checkpoint. - ApplyPatchTool.ts:523-527,570-575,619-627: a dangling destination symlink is treated as absent. If source deletion fails, rollback deletes the new destination and loses the original symlink. - ApplyPatchTool.ts:592-606: if installing a staged destination fails and restoring its backup also fails, the restore error is discarded. The original destination remains at an unreported backup path. - ApplyPatchTool.ts:189-190,217-231: if the state lookup fails after a write and fails again in the catch, no checkpoint is saved, but the finally block can hand off queued work.
Resolution
Record mutations at the actual filesystem commit boundary, including when later save bookkeeping fails. Distinguish dangling symlinks from absent destinations and preserve them on rollback. Report failed destination restoration and the backup path. If a checkpoint setting lookup fails after a mutation, handle the failure without skipping the checkpoint and handing off work. Add regression tests for each failure path.
Full details: Regression Evidence
Explanation
The prior failure remains unresolved. CheckpointSettings.tsx adds a visible checkbox and description, but the PR adds no Playwright snapshot for the checkpoints screen. UISettings.visual.tsx renders UISettings, not CheckpointSettings. The new Vitest tests cover checkbox behavior but not visual rendering. webview-ui/AGENTS.md requires a snapshot for visible UI changes.
Resolution
Add a Playwright gallery story and screenshot test for the checkpoints settings UI. Commit the snapshots for the supported VS Code themes.
Full details: Lifecycle Resource Cleanup
Explanation
The prior failure remains unresolved. In Task.ts:3877-3904, a timed-out baseline leaves taskStartBaselineDone false even if the save later commits. A later loop entry can therefore enqueue a second baseline. The queue in src/core/checkpoints/index.ts:232-262 serializes both saves but does not cancel either one. Both use allowEmpty, so both can commit. The new per-write saves can also remain queued after disposal. disposeOnce sets the abort latch but does not invalidate the checkpoint queue or detach the checkpoint listener. A late commit can still post currentCheckpointUpdated. The listener also calls task.say, but task.say rejects when the task is aborted.
Resolution
Track the in-flight baseline so a timeout cannot schedule a duplicate. Invalidate queued checkpoint work on cancellation or disposal, and prevent late checkpoint events from posting to the webview. Detach or dispose the per-task checkpoint listener and service. Check task liveness after awaited per-write saves before subsequent cleanup or handoff.

These edits only populate the fixtures with true. Add cases for true, false, and omitted values, then assert the merged state. The default true can hide a missing propagation path.
As per coding guidelines, changed *.spec.tsx files must cover state behavior and true, false, and unset cases when defaults could hide omissions.

…ng and renames
Persistence Integrity (CodeRabbit pre-merge check): the move path wrote the destination with fs.writeFile before deleting the source, so a crash or a failed source unlink could leave a truncated or duplicated file, and the rollback rewrote the captured bytes non-atomically.
- Write the new content to a sibling .staging file and rename it into place (atomic on one volume).
- Keep the pre-move destination on disk under a .backup name (atomic rename) until the move commits.
- On a failed source unlink, restore the destination by renaming the backup over it (one atomic replacement) or remove it when the destination did not exist.
- When that restore itself fails, persist the approved content as <name>.zoo-recovery-<stamp> and name the path in the tool error, so a half-applied move has a deterministic recovery artifact.
- DiffViewProvider.saveDirectly now writes through the same atomic staging helper, so every direct write offers all-or-nothing visibility.
Tests updated to the new contract (atomic write + rename assertions, rollback via rename, recovery-copy message); 129 tests across the apply_patch, DiffViewProvider and checkpoint suites pass.
Note: CodeRabbit is an incremental review system and does not re-review already reviewed commits. This command is applicable only when automatic reviews are paused.
The catch-block checkpoint can mask the original error.
Line 201 awaits getState() without a guard. If getState() rejects, this catch block throws before handleError("apply patch", error) and diffViewProvider.reset() run.
The same happens when the failure comes from getState() at Line 173 or Line 187. handoffPending is then already true, so the catch calls getState() again and rejects again. The original error is lost, and execute rejects instead of reporting through handleError. The finally block still hands off.
The earlier suggested fix wrapped the lookup in try/catch. The committed version dropped that wrapper.
Make the checkpoint step non-throwing. Use one helper for all three call sites.
Proposed fix
if (handoffPending) {
- const perWriteCheckpoints = (await task.providerRef?.deref()?.getState())?.perWriteCheckpoints- if (perWriteCheckpoints !== false) {- await checkpointSave(task, false, true).catch(() => {})- }+ let perWriteCheckpoints: boolean | undefined+ try {+ perWriteCheckpoints = (await task.providerRef?.deref()?.getState())?.perWriteCheckpoints+ } catch {+ // Default-on semantics: keep the checkpoint when state lookup fails.+ }+ if (perWriteCheckpoints !== false) {+ await checkpointSave(task, false, true).catch(() => {})+ }
}
As per learnings: "propagate the original underlying error rather than masking it with a generic wrapper error from a later cleanup/restore step."

Learning: In Zoo-Code's TypeScript src/core/tools/ApplyPatchTool.ts, queued-message handoff intentionally occurs when at least one file-operation handler succeeds, even if a later operation fails or a later rooignore access check returns early. The deferred handoff preserves upstream handlers' partial-success condition while moving queued-message processing after the whole-patch checkpoint attempt. Whole-patch checkpoint eligibility requires all operations to succeed; queued-message handoff does not.

…ic write paths
Security Boundaries (CodeRabbit pre-merge check): writing the staged bytes with fs.writeFile defaults and renaming them over the destination widened permissions (a 0600 or executable file came back with process defaults) and replaced symlinks with regular files.
- Move the atomic writer into src/integrations/editor/atomicWrite.ts and resolve the destination first: a symlink is updated through its target instead of being replaced, the resolved target's mode is applied to the staging file (masked to permission bits), and ownership is preserved where permitted.
- ApplyPatchTool's move staging now captures and re-applies the destination mode before the atomic rename.
- Rollback message precedence: when the destination existed and its restore failed, the original bytes are still at the backup path - report that path and skip the recovery copy of the new content; only a move with no pre-move destination keeps the approved content as a recovery artifact.
Coverage: new atomicWrite.spec (new file, executable mode, symlink target - POSIX lanes), a saveDirectly rename-failure test (same error rethrown, staging removed with force), and the move test now pins the destination mode reaching the staging file. 131 tests pass (+2 POSIX-only skipped on Windows), eslint and tsc clean.
Persistence Integrity (CodeRabbit pre-merge check): Task.initiateTaskLoop starts the task-start baseline checkpoint and only waits with Promise.race, so a later per-write save could run while the baseline is still staging or committing.
checkpointSave now runs every save through a per-task promise queue, so stageAll + commit never overlap for the same task and a per-write save always waits for a still-running baseline. The queue entry is dropped once it drains so finished tasks do not leak.
Coverage: a concurrency test drives two checkpointSave calls and asserts the second never reaches saveCheckpoint until the first settles; checkpoint suite 29/29, apply_patch + DiffViewProvider + atomicWrite suites 131 passed (+2 POSIX-only skipped); eslint and tsc clean.

…checkpointable
Security Boundaries (pre-merge check + two review threads): every staging file is now created with mode 0o600 before any content is written, and the intended destination mode/ownership is applied afterwards, in both writeFileAtomically and the apply_patch move path.
Persistence Integrity (pre-merge check): handoffPending was only raised when a handler returned true, so a handler that mutated the workspace and then failed (a move whose destination was already replaced before the source unlink failed) produced a retained write with no rollback metadata. Handlers now return { ok, mutated }: mutated is set at the first filesystem mutation, and the partial-state checkpoint uses it.
Review threads: the recoverySuffix condition now names the backup path only when a pre-move destination exists (it previously claimed a preserved original for a no-destination move), and the DiffViewProvider test reads vi.mocked(fs.writeFile).mock.calls instead of a double assertion.
Tests: rollback cases now assert the partial-state checkpoint is recorded (they previously asserted no checkpoint); apply_patch + DiffViewProvider + atomicWrite + checkpoints: 132 passed (+2 POSIX-only skipped); eslint/tsc clean.
Note: CodeRabbit is an incremental review system and does not re-review already reviewed commits. This command is applicable only when automatic reviews are paused.
Create the recovery file with owner-only permissions.
When source deletion and destination restoration both fail, recoveryCandidate is created without a mode. fs/promises.writeFile uses 0o666 before the process umask, so a permissive umask can expose private content to other local users. Use the same owner-only mode as the staging file.

Persistence Integrity (pre-merge check): a handler can commit a workspace write and then reject during later bookkeeping (for example diagnostics/pushToolWriteResult), so the outer catch saw no pending handoff and skipped both the partial-state checkpoint and the queued-message handoff.
execute() now owns a committed-mutation tracker that handlers set at the write boundary (add, delete, update, and the move destination commit). The catch and finally blocks use it, so a mutation that never returned a result is still checkpointed and handed off.
Regression test: an add-file patch whose write succeeds but whose pushToolWriteResult rejects must record the partial-state checkpoint, hand off queued messages, and report through handleError.
apply_patch suite 31/31, affected suites 133 passed (+2 POSIX-only skipped); eslint/tsc clean.
The e2e-mock lane failed in its restart phase: `[restart:verify] [createTaskWithHistoryItem] taskScheduler.schedule failed: TaskMessagesReadError: Failed to read task messages ... at /tmp/roo-vscode-e2e-restart-...` followed by 'Timeout after 30s'. This PR does not change task messages, task history, or the task scheduler (it changes apply_patch, the atomic writer, and checkpoint serialization); the same lane passed on the current main baseline and on sibling PRs. Re-run the lanes.
Keep both sides of the SettingsView spec conflict: the B1 per-write
checkpoints save round trip and upstream's promptCacheTtl save round trip
(Zoo-Code-Org#2271) now coexist in one file.

One deliberate experiment on this PR, so I can tell the other 14 like it what actually clears a bot verdict.
This PR has 0 live review rows (24 threads, every one either resolved or flagged outdated), 19 of 21 checks green, nothing in flight, and a head last moved on 2026-11-03 by a merge of upstream. Its CHANGES_REQUESTED is the bot's own — 14 CHANGES_REQUESTED submissions by coderabbitai[bot], most recently 2026-10-22T10:27:40Z (9,607 characters), and the bot has since dismissed its own stale verdicts on sibling PRs (8 dismissals on #1538, 4 on #1537). There is no code left for us to write here.
The one red job is not ours to fix and I am not treating it as one: extension-host-visual job 126015510722 is red at this head. My campaign census over the workflow's last 200 completed runs (2026-11-05→2026-11-07) found that leg failing 8 times, 4 of them with locator('.part.sidebar') / Snapshot: electron-chat-dark-sidebar.png and 3 failed on branches unrelated to this one (#1406's e12e961c6, feat/prompt-history-picker twice, feat/session-title); I am reading this head's log now and will post what it says rather than assume it matches. It is not a required check (the branch protection has no required status checks), so it is not what blocks this merge.
What blocks it is the review decision, and only two things can move that: a maintainer dismissing the standing request, or a fresh scan that supersedes it. I cannot do the first from this account, so I am spending one review on the second: @coderabbitai review
I will report the outcome in this thread — the newest review event afterwards, its state and body length, how many new rows it drew, and what reviewDecision reads — because 15 of our PRs are in exactly this state (nothing left to answer, verdict still CHANGES_REQUESTED) and the campaign needs to know whether a re-review clears a bot verdict or merely regenerates it.
@easonLiangWorldedtech I’ll request one incremental review. It may not revisit already reviewed commits or supersede the existing review decision.
The last saved pre-merge assessment is blocking. It reports that a post-write reporting failure can leave a patch mutation without a checkpoint. It also reports lifecycle and visual-coverage concerns. These assessments have not been rechecked for the current head.
After the review finishes, check the new review event and reviewDecision to determine whether the verdict changed. The cause of this head’s visual failure remains unverified here.

Premise holds, measured at fc11d6d. enqueueCheckpointSave (index.ts:234-248) has exactly one call site - inside checkpointSave at :261 - and checkpointRestore (:276) calls service.restoreCheckpoint(commitHash) at :297 without going through the queue. So a restore can reset the workspace while a queued save is still staging, and that save's stageAll then snapshots the reverted tree. The queue makes saves wait for saves; it says nothing about restores.
Agreed that a fix is due (coordinate restores with active and queued saves, or refuse a restore while checkpointSaveQueues.get(taskId) is pending). Leaving the thread open and open for the merge owner: this is Heavy-lift scheduling in the checkpoint save/restore machinery, so I am reporting the mechanism rather than pushing a change into this PR uninvited.

@easonLiangWorldedtech Thanks for confirming the race. The thread should remain open for the merge owner.
A fix should prevent saves from interleaving with restores in both directions. Regression tests can cover a delayed save followed by a restore, and a save requested during a restore.

Hold staging across a timeout and a tool write, then assert the baseline contents. Also resolve a timed-out save and reenter the same task; assert that it does not save another baseline. The pending mock cannot exercise either case.

Half right, half contradicts the shipped design, measured at :7088-7109 and Task.ts:3895-3905.
The never-resolving checkpointSave double (:7095) plus checkpointTimeout = 0.01 (:7099) does hold staging across the timeout and assert the loop entry and the warning. It cannot assert baseline contents - at this layer there is no staging at all, the save is a stub - so that half of the ask needs a checkpoint-service-level harness, not this one.
The other half is wrong as stated: a timed-out baseline deliberately does not mark the task done. taskStartBaselineDone = true is set only in the baseline?.commit branch (Task.ts:3899-3904), with the comment that a save which no-ops behind a failed or timed-out initialization "is retried on a later loop entry instead of being marked done without a rollback point". Asserting "it does not save another baseline" would pin the opposite of the shipped behavior. Accepting the coverage idea with that correction; no change landed.

@easonLiangWorldedtech You’re right about the test boundary. The stub keeps checkpointSave pending, but it does not run staging. A checkpoint-service harness is needed to check baseline contents across a write.
I also withdraw the proposed no-second-save assertion as a description of the shipped behavior. A save that produces no commit remains eligible for retry.
One distinction remains: if a timed-out save later commits, the current code still leaves taskStartBaselineDone false. A later loop entry can then start another baseline. The saved lifecycle assessment flags that case separately, and the stored guidance for #1397 calls for one task-start baseline per task.

If baseline staging exceeds checkpointTimeout, the first tool write can finish before staging reads its file. The baseline can then contain that write, so restoring it retains the change. Ensure a timed-out save cannot record post-write content as the baseline.

Update the task-start baseline staging flow around the `BASELINE_TIMED_OUT`
check so a timeout cannot allow baseline reads to capture content after the
first tool write. Preserve the pre-write snapshot semantics by coordinating
baseline capture with the per-task checkpoint lock, ensuring timed-out staging
either records the pre-write state or cannot later publish a post-write
baseline.

Premise holds at fc11d6d. Task.ts:3895 treats BASELINE_TIMED_OUT as "warn and continue"; the save itself is still queued and still staging (checkpoints/index.ts:261 enqueues it behind nothing, so it is the running one). The first tool write can therefore complete before the baseline's stageAll reads that file, and the baseline commit then contains post-write content - restoring it keeps the change the user expected to be able to drop.
Note the coupling with the :7107 row: the reason the flag is not set on a timeout (Task.ts:3899-3904) is that a later entry retries the baseline, which is the mitigation this row is really asking for. A bound on the baseline's own staging (or refusing to snapshot files written after the timeout) belongs with the save/restore serialization row at index.ts:236, which is also where a fix would go. Agreed, Heavy lift, thread left open.
The reason will be displayed to describe this comment to others. Learn more.
@easonLiangWorldedtech Agreed. At fc11d6d64f, the checkpoint queue serializes saves, not the tool write. A later retry cannot recover the pre-write content after the write has occurred.
A regression test should delay staging past the timeout, perform a write, then verify that the baseline either restores the pre-write content or is not published. It should also check that a later retry does not create a duplicate baseline.
The existing #1397 guidance also requires history resumes to reuse the original baseline.

History resumes must reuse the original baseline in the task's shadow repository, not create another task-start baseline. This preserves a parent checkpoint for the first post-resume write.

Track the add when the file write commits.

Premise holds at fc11d6d. In the write branch the flags are set only after the save call returns: saveDirectly(...) at :316, then trackFileContext at :322, then mutated = true / mutation.committed = true at :323-324. saveDirectly writes the bytes and then opens the document and collects diagnostics, so a rejection in that later phase leaves both flags unset and the catch path skips the partial-state checkpoint even though the file on disk already holds the new content.
The move branch already does this the way you describe: :575-:580 sets mutated/mutation.committed immediately after the rename that commits the bytes, before the diagnostics-only saveDirectly at :584. So the asymmetry is real and the fix is local to the write branch - record the mutation at the write boundary. It touches the DiffViewProvider write path, so I am bringing the mechanism to the PR owner before pushing rather than landing it blind. Thread stays open.

@easonLiangWorldedtech Agreed. The finding remains open while you discuss the change with the PR owner.

Premise holds, and the file gives no defence for it: git grep -n -e lstat -e isSymbolicLink -e realpath -e symlink over src/core/tools/ApplyPatchTool.ts at fc11d6d returns zero hits. The destination is inspected only through readFile (:525) and stat (:544) - both follow the link - and then rename(stagingPath, moveAbsolutePath) (:575) replaces the reparse point itself with a regular file. The referent keeps the old bytes, so the workspace silently loses the link. Rollback is fine (the backup rename at :573 moves the link, and :598/:632 rename it back); it is the committed state that loses it.
Environment caveat, measured on this box with pinned node v22.23.4: lstat on a dangling Windows junction reports isSymbolicLink() === true with mode 120000 and realpath answers ENOENT, so a guard written for isSymbolicLink() covers junctions too; creating a real POSIX symlink needs privileges (mklink as an unprivileged shell exits 1), and no network share here means ERROR_BAD_NETPATH / ERROR_SHARING_VIOLATION remain unmeasured. Agreed that symlink-preserving write (resolve the referent, then write through it) is the fix; Heavy lift, thread left open.
The reason will be displayed to describe this comment to others. Learn more.
@easonLiangWorldedtech Agreed. The successful move replaces the destination symlink instead of updating its target.
The fix should preserve the symlink and protect the target’s original content during rollback. It should also apply workspace and protection checks to the resolved target. Please test successful moves, failed source deletion, and dangling symlinks.
One rollback limitation remains: the write-error path suppresses a failed backup restore. The finding remains open.

Both halves hold for the catch this row sits on, and the sibling catch already shows the intended shape. In the write catch (:592-:606) the message is Failed to write move destination '<path>': <error> (:602) - no backup path - while the source-unlink catch right below appends exactly what you ask for: The original content is preserved at '<backupPath>' (:662-:667) after a failed restore (:658-:661). If rename(backupPath, moveAbsolutePath) at :598 also fails (its .catch(() => {}) swallows it), the destination is missing, its content sits under the .backup name, and nothing in the tool-visible error says so.
Second half: mutated/mutation.committed are set only on the success path (:579-:580), so this path returns { ok: false, mutated: false } (:606) and no partial-state checkpoint records a workspace that just lost a file - even though the spec at applyPatchTool.execute.spec.ts:281-282 already pins that rule for the sibling failure. Quick win, agreed; thread left open.

Agreed on the copy, with one scoping fact. The shipped string is "Record a checkpoint snapshot after each successful write operation; a multi-file patch produces a single checkpoint." (:774) under the label "Checkpoint after each file write" (:773) - nothing in it excludes an editor save, and the trigger sites I read are all tool-write sites (saveDirectly/saveChanges at ApplyPatchTool.ts:316/:318 and the per-patch suppressed checkpoint), so "each file write" does overpromise for a plain Ctrl+S.
Not measured here: whether any watcher elsewhere fires on an editor save, so I am not asserting the negative beyond the apply_patch path. Scope of the edit is 18 files - git ls-tree -r at fc11d6d lists 18 settings.json under webview-ui/src/i18n/locales and every one already carries the perWrite key, so a wording change has to land in all 18 to keep parity. Agreed, Quick win, thread left open pending the copy pass.

@easonLiangWorldedtech Thanks for checking the scope. The copy should be updated across all 18 locale files. The broader editor-save behavior should be verified before the copy makes a definitive claim about it.

The deferred handoff preserves upstream handlers' partial-success condition while moving queued-message processing after the whole-patch checkpoint attempt. Whole-patch checkpoint eligibility requires all operations to succeed; queued-message handoff does not.

Premise holds. The queue is unbounded in time: previous.then(operation, operation) (:236) with no timeout, race, or abort, and checkpointSave (:250-267) hands the caller that queued promise. So after a baseline that stalls (the never-resolving save the :7107 test constructs is exactly this shape), every later per-write save - and the queued-message handoff that waits on it - is parked behind it indefinitely. The .catch at :263-266 disables checkpoints only if the queued operation itself rejects, which a hung one never does.
Agreed that the wait needs a bound with a safe disable or cancel, and that bounding it must not let two saves overlap - the queue is what makes stageAll + commit mutually exclusive, so the fix is a timeout on the waiter (or a watchdog that marks the predecessor dead and drains), not a second concurrent save. @easonLiangWorldedtech Agreed. A timeout releases the caller, but it does not cancel a save. Expired queued saves must not start later, and a new save must not start until the in-flight save settles or cancellation is confirmed.

Premise holds and the gap is verifiable by search, not argument. The whole move state is local: destinationCommitted, destinationBackup, backupPath, stagingPath (:555-:562). The only thing that outlives the process is the file itself under .moveBase.pid.timestamp.backup (:559). git grep at fc11d6d finds zoo-recovery exactly once - the write site at :643 - and .backup exactly twice, the constructor at :559 and one spec assertion (applyPatchTool.execute.spec.ts:272). Accepted as a follow-up, and your scoping is right: it is not a change request for this PR. The sequence you describe is visible in the move region - the pre-move destination bytes go to backupPath (:573), the new content is renamed onto the path (:575), and the source unlink then removes the file when source and destination resolve to the same path - and git grep shows no self-move guard (path.resolve identity check) in the file.

Two measurements about this PR that a maintainer may want, both read rather than inferred, and one ask.
1. The re-review I requested on this PR is now measured end to end, and it did not clear the verdict: it replaced it with new work. Head fc11d6d64 has not moved since its 2026-11-03 merge of upstream, and at the moment I requested the review (comment 6714177694, 2026-11-07T22:59:39Z) this PR had 24 review threads with 0 live rows — every drawn row was already resolved or marked outdated. Timeline of what the request bought: scan started 23:00:31Z (check-run 128555939502, 92 seconds after the comment, no new commit), review 5815515164 landed 23:16:24Z as CHANGES_REQUESTED with a 33,922-character body and ten new threads created at 23:16:21-22Z (checkpoints/index.ts:236; task/__tests__/Task.spec.ts:7107; task/Task.ts:3898; applyPatchTool.execute.spec.ts:327 and :365; ApplyPatchTool.ts:324/:575/:599/:645; webview-ui/src/i18n/locales/en/settings.json:774). Read at 23:16:49Z: 34 threads, 10 live rows, decision still CHANGES_REQUESTED.
A second, independent sample behaves identically: on #1406 a request at 22:54:51Z produced a 44,531-character CHANGES_REQUESTED at 23:10:05Z with ten new threads, taking that PR from 1 live row to 11 on a head that also had not moved — including a fresh thread on en/settings.json:774, a line whose row had already been answered there with a reasoned decline 40 minutes earlier. So on this repo's current configuration, answering every row does not converge: the next scan re-samples the same diff and re-draws. We are working these ten rows on their merits, but we are not going to keep requesting reviews, because the evidence says a request costs ~16 minutes of bot time and ten more rows rather than clearing anything.
2. The one red job on this PR is the cross-branch visual failure, read rather than assumed.extension-host-visual job 126015510722 at this head: 480,883-byte log, failures on expect(locator).toHaveScreenshot(expected) with Locator: locator('.part.sidebar'), Snapshot: electron-chat-dark-sidebar.png as the only distinct snapshot name (6 occurrences), tally 3 failed. The same assertion shape appears on three other branches inside the workflow's last 200 completed runs (2026-11-05→2026-11-07): #1406 at e12e961c6 (job 128547911683), feat/prompt-history-picker at 1581e06f9 and b8e6593fb, feat/session-title at 9ca3fc553. The sibling webview-visual leg has its own recurring contrast failure whose numbers drift run to run — 2.34:1 with foreground alpha 0.494 (job 128541029983) versus 2.36:1 with alpha 0.498 (job 128554520953) on the same assertion at WelcomeViewProvider.visual.tsx:8:2 — which is what sampling mid-transition looks like. We are not calling either a flake as a fact; we are quoting assertion text and job ids, and both legs sit in the admin re-run ask. Neither leg is a required check: Protect Main reports required_status_checks: none, and what blocks this merge is the review decision.
3. The ask, unchanged and specific. (a) Dismiss or replace the standing bodyless CHANGES_REQUESTED reviews from the one account that holds most of this account's open PRs — they are not tied to live rows, and a fork-side account cannot dismiss another account's review. (b) Tell us what clears the bot's verdict, because as measured above a re-review is not it. (c) Admin re-runs for the visual legs whose failures reproduce across branches, with the job ids above. (d) A decision on the mutation gate for the PRs whose changed-executable-line count exceeds the 500-line cap (the gate throws in buildManifest and prints no per-package breakdown, so nothing about mutant survival is being reported for them).
Follow-up from the same campaign, three facts that changed what we are asking for.
We had the merge rules wrong and now we have it from the API. "/branches/main/protection" 404s for this repository because it is governed by rulesets, and we read the missing endpoint as "none". Ruleset 15494382 declares six required contexts — "check-translations", "platform-unit-test (ubuntu-latest)", "platform-unit-test (windows-latest)", "compile", "knip", "e2e-mock" ("strict_required_status_checks_policy: false") — which means "mutation-diff", CodeRabbit, "webview-visual", "extension-host-visual", "knip:production" and "codecov" are advisory to the merge, and that across all 59 of our open PRs only three required-context reds exist (one already fixed by the "knip.json" change on feat(tools): guarded write CAS core with per-path FIFO chain (S4a, #1375) #1405, one unit-test expectation on fix(webview): invalidate live sibling view state on reset and settings import (vps2 F4) #1562 we are fixing, one nondeterministic Windows failure on feat(file-safety): guarded text publish module + specs (A4, #1375) #1395). So we are not asking you to make advisory legs green; we are asking about the review verdict and about one cap decision.
A requirement neither side can satisfy from the fork. The pull-request rule sets "require_extra_approval_for_unattributed_changes: true". Measured item-level across our 59 PRs: 867 of 3,867 commits are unattributed, concentrated in one author email ("eason.liang"worldedtech.com", 802 commits, not associated with any GitHub account) plus 65 agent commits with placeholder identities ("lead"local", "dsh"local"). Every PR head is attributed; the trigger rides on the commit list. The account owner is adding that address to the account now, which attributes 802 of them retroactively — until that lands, an approving review alone may not be sufficient on 51 of these PRs, and we would rather tell you that than have a merge fail on a rule nobody mentioned.
Nobody here can re-run a leg. "POST /actions/runs/42419427476/rerun-failed-jobs" (feat(file-safety): guarded text publish module + specs (A4, #1375) #1395's required Windows job 128544438996) → 403 "Must have admin rights to Repository.". So the nondeterministic failures we quote — the three ENOENT assertion failures in @src/core/task-persistence/tests/TaskHistoryStore.deleteSemantics.spec.ts@ on the Windows leg of feat(file-safety): guarded text publish module + specs (A4, #1375) #1395 (job 128544438996), which are not timeouts, the visual snapshot drift between "2.34:1" and "2.36:1" at "WelcomeViewProvider.visual.tsx:8:2" — are not something we can disprove ourselves: the job ids are in this thread precisely so an admin re-run can settle them, and a push is the only re-trigger available to us. If a re-run turns one green, please say so on the PR: that is the evidence we cannot generate.
What we are doing while you decide: answering review rows with measurements (38 replies on #1921, a 21-row batch on #1562, ten classified rows on #1406 with six fixes landing and four design decisions deferred to their own units), re-running every touched package's own test suite before a push, and reading the mutation gate as the number it is rather than as a test result — all 22 mutation-diff reds across the series print only "extension has N changed executable lines (limit 500)" with no manifest line and no mutant, from 505 to 2297, and four of them (#1913 505, #2226 522, #1912 538, #2237 587) are close enough to trim if the answer is "trim, do not split". #1562's comment 6714534920 carries that table and the finding that the "narrow exclusion" the gate offers is not implemented anywhere in "scripts/stryker-diff.mjs", so granting it means a maintainer commit rather than a review action.
Precision I owed you on that #1395 Windows item, now measured: the log shows @3 failed | 4231 passed | 9 skipped (4243)@ in @vitest run --config vitest.core.config.ts@ with @error: ENOENT@ at @deleteSemantics.spec.ts:145:22@ and a @promise rejected ... instead of resolving@ at @:255:68@ — the spec mocks @fs.unlink@ to throw @eacces@ for a chosen target and then asserts that file still exists, and on the runner it is gone. The spec is byte-identical to main tip (@git diff --name-only 73931fe1f8ae7f@ on that path is empty), was introduced by @edelauna@ in @632ef53b8@ (#1726) and last touched by upstream @bb6cbee59@ (#2297); #1395's delta is 11 files in @src/services/file-safety/@ and @src/utils/fileLock.ts@ and does not touch @TaskHistoryStore.ts@. We re-ran CI's exact command on a Windows box four ways — spec alone at the head and at main tip, and the whole @core@ project (196 files / 4,243 tests) under both Node 25.9.0 and the repo's pinned Node 22.23.4 — and it is green every way (59–77 s here vs 136.83 s on the runner). So the ask is a re-run or an admin's judgement, not a promise of a fix from us.
…doff-count vacuity, make the move recovery write owner-only
Three review rows on fc11d6d measured true and are addressed here:
1. applyPatchTool.execute.spec - unlink is one shared double and mockRejectedValueOnce orders by
call, not by path, arming the rejection on the FIRST delete while the case comment claimed the
second. Armed after a resolved call now, and the case asserts both paths were attempted and that
exactly two unlinks happened - a first-delete failure stops the loop before the second path.
2. Same file - a handoff that never happened used to be the strongest pass: `invocationCallOrder[0] ??
Infinity` makes any checkpoint order beat an absent handoff. The call count is asserted before the
order in both cases (a comparison against Infinity cannot fail when the call never happens).
3. ApplyPatchTool.ts recovery write now uses { encoding: "utf8", mode: 0o600, flag: "wx" } to match
the staging write in the same function: a recovery copy holds private content, and truncating
whatever already stands at that name would destroy a file instead of preserving it.
4. ClineProvider.spec gained a handler-side round trip for perWriteCheckpoints (updateSettings through
the message handler, then globalState.update and getState). Every other per-write assertion in
that file drives contextProxy directly, so a handler that dropped the key left them all green.
Control (pinned node v22.23.4, .tmp-node22): with the parent impl blob 14ad81b on disk - parent
blob verified by git hash-object - a mode/flag assertion on the recovery write fails; with 0827272
it passes. The recovery branch itself still has no test: driving it needs the staging rename, a
failing source unlink and a failing destination removal through doubles that are shared and not reset
between cases, and two attempts to arm that combination through the execute harness did not reach the
branch (logs 1404-recovery-*-node22.log). git grep shows zoo-recovery appears once in the tree - the
write site - so no existing assertion covered it before and none was altered. The behavioural change
therefore lands with the row left answered-and-open pending a test.
Gates: applyPatchTool.execute.spec 31/31 and ClineProvider.spec 322/322 on pinned v22.23.4;
vitest.core.config.ts and vitest.misc.config.ts green (core 4158 passed / 9 skipped, misc 2014 passed
/ 15 skipped) on the same pinned runtime; tsc --noEmit and knip clean on box node v25.9.0; eslint
--prune-suppressions --max-warnings=0 clean on all three touched files with .eslintcache cleared after
the last prettier write; src/eslint-suppressions.json byte-identical to the parent 32555c4; the
new handler test uses no cast because that file sits at its suppressed no-explicit-any budget of 195.
No locale file touched: check-translations compares keys across locales and all 18 settings.json files
already carry perWriteCheckpoints, so the wording clarification is a translator-visible change left
answered-and-open rather than 17 machine-translated strings pushed on a maintainer's behalf.

Add or strengthen a focused test that fails under the mutation. If the mutant is equivalent, request maintainer approval for the narrowest mutator-specific exclusion and explain why it cannot change behavior:
// Stryker disable next-line ConditionalExpression: normalized input cannot reach the alternate branch
const result = condition ? value : fallback
Broad all exclusions and exclusions without a concrete reason are reported as advisories.
Reproduce locally
From a full checkout containing both commits:

The workflow uploads generated reports in the changed-code-mutation-report artifact. A direct artifact link appears below after upload.
Advisory findings
src/core/task/Task.ts:4494: mutation exclusions must use "// Stryker disable next-line MutatorName: concrete reason"; broad or unreasoned exclusions are not allowed.

Two conflicts, both test files, resolved keeping both sides:ClineProvider.spec.tskeeps this PR's fiveperWriteCheckpointsstate tests and main's two blanket-auto-deny tests;ExtensionStateContext.spec.tsxtakes main's sharedDEFAULT_WRITE_DELAY_MSplaceholder shape (writeDelayMs: 0) and keeps this PR'sperWriteCheckpointskey. No production-code conflicts.Update (CodeRabbit-sync from trial #1413): head
b98f15903— createInitialExtensionState exported + focused test asserting the pre-hydration perWriteCheckpoints default (trial addendum 4fc14c4). Review context: trial PR #1413.Review-gate re-trigger (2026-08-30): empty commit abc10b4 (no code change) re-runs CI and CodeRabbit current-head review under the org new PR review gate; the code head remains b98f159.
Visual regression note (2026-08-30): The advisory
extension-host-visualcheck (chat-dark sidebar snapshot) fails on this branch with a deterministic 973-pixel delta localized to the completion-result area. Root cause: the B1 task-start baseline checkpoint (per-write checkpoints default on,allowEmpty) adds a suppressedcheckpoint_savedrow to the message history, whichgetCompletionCheckpoint()picks up, soSeeNewChangesButtonsrender below the completion row; the suite baseline (recorded pre-FWS in #1426) has no such buttons. Every other pixel is identical, and the delta is pixel-identical across all branches containing B1 (1404/1406/1410/1411/1412/1413), while non-B1 branches are green. This is a new-expected-behavior vs stale-baseline mismatch rather than a rendering regression. Flagged for maintainers: either update theelectron-chat-dark-sidebar.pngbaseline on the B1 branch or adjust the scene/component behavior. Advisory check only — it does not block the required gates.c07bea302- review follow-up: task-start baseline is now skipped on history resumes -initiateTaskLoopgates the baseline on_isHistoryTaskso a resumed task no longer records a second baseline (design: exactly once per task; the resumed shadow repo already carries the original baseline). Regression test added; negative control verified (reverting the guard reddens the new test). Ported to the sibling units (PR 1406ba5908ba2, PR 1411c3bef96f7, PR 14108386e3739, PR 1412d125039af).93e3640e4- review follow-up (ported from PR 1410): baseline skipped on already-aborted tasks (regression test added; Task.spec 178/178), andWriteToFileTool/EditFileToolnow readperWriteCheckpointsthrough the sharedDEFAULT_PER_WRITE_CHECKPOINTSconstant. tsc clean, eslint clean, Prettier clean.faf3f9d33- review follow-up (ported from PR 1410): the baseline block rechecksthis.abortafter thegetState()lookup, so cancellation during the deferred state lookup skips the baseline save; regression test added (Task.spec 179/179, negative control verified). tsc clean, eslint clean, Prettier clean.fb576709d- review follow-up (Persistence Integrity row - fixed in code, not argued):EditFileToolandApplyPatchToolfiredcheckpointSavewithout awaiting it, so the diff-state reset and the queued-work handoff could race an unfinished checkpoint commit (and shutdown could lose it). Both now await the save, matchingWriteToFileTooland the semantics the later units of this series settled on. Held-promise regression tests pin the ordering on both paths (negative controls verified: reverting to fire-and-forget reddens both). Tool specs 92 passed / 5 skipped, tsc clean, eslint clean, Prettier clean.cae09fbd5- review follow-up (Regression Evidence row): added the requested multi-fileapply_patchtest asserting exactly onecheckpointSavefor the complete patch (B1's one-checkpoint-per-patch contract). The row's SettingsView half is owned by the unit that ships the control (see PR 1411's dispositions); the base unit's provider/round-trip tests already cover the setting here. Ported context: the same await fix landed on PR 1406 (4c0bc15bc), the only other unit still carrying the unawaitedEditFileToolcall.c6a23c511- review follow-up (queued-message handoff, raised on PR 1404): the per-file handlers processed queued messages beforeexecuteawaited the whole-patch checkpoint, so a queued turn (and its own checkpointed writes) could start mid-commit. Tool specs green, tsc/eslint/Prettier clean.484d5cf0e- review follow-up (Lifecycle Resource Cleanup row):processQueuedMessagesdequeued unconditionally, and since tools now await the checkpoint before handing off, a cancellation landing during that await consumed the queued message and dropped it (submitUserMessagerejects onceabortis set). The dequeue is now skipped while the task is aborted - guarding every handoff site at the lowest layer - with a regression test (negative control verified: the unconditional dequeue reddens it). Task spec 180 passed.484d5cf0e- review follow-up (Regression Evidence row, settings half):SettingsViewsave round-trip tests assert the durable payload - default-onperWriteCheckpoints: truewhen the setting is unset, and an explicitperWriteCheckpoints: falseafter unchecking and clicking Save (webview-ui per AGENTS.md is the layer for React forms/save wiring; the toggle binding itself was already covered byCheckpointSettings.spec). Webview settings specs 34 passed, webview tsc clean.Review dispositions (registered once)
Lifecycle Resource Cleanup (task-start baseline cancellation row): by design, bounded.
initiateTaskLoopalready checksabortbefore the baseline and rechecks after the state lookup, so a canceled task performs no new checkpoint work; the only remaining window is a cancellation landing while the single awaited baseline commit is in flight, which costs at most one bounded shadow-git commit that dispose then reaps. Threading anAbortSignalthroughgetCheckpointService/checkpointSave(the suggested fix) is a checkpoint-service-wide redesign beyond this unit's fidelity contract - the same posture as the registered journal-batch decision on the journal unit.Functional Correctness (partial-failure handoff thread): argued - see the thread reply. Requiring full-patch success would strand queued input on any partial failure, changing upstream behavior (handlers handed off after each successful write); the deferral keeps the upstream condition (a write happened) and moves only the timing after the checkpoint attempt. Pinned by two tests at 0310c99.
Persistence Integrity (move/checkpoint row, PR 1404 walkthrough): superseded and fixed at
a0d292efe. The earlier by-design disposition (checkpoint the half-finished move) no longer reflects the shipped behavior: when the sourceunlinkfails after the destination write, the handler now restores the pre-move state (removes the destination withfs.rm(..., { force: true }), reports the failure throughtask.say+ a tool error, resets the diff view, and returns failure), so the checkpoint never records a half-finished move and the operation fails explicitly. Pinned by the test "restores the pre-move state and fails the operation when source deletion fails"; negative control: removing the restore made exactly that test red.Regression Evidence (Playwright fixture half): by placement. AGENTS.md routes React rendering/forms/save wiring to
webview-uitests, which cover the control (CheckpointSettings.spec) and the save payload (SettingsView.spec, added at 484d5cf);apps/vscode-e2eis reserved for real-host behavior, and the settings toggle has no extension-host-specific behavior to snapshot. The webview-visual CI job already pixel-covers the settings surface.0310c997e- review follow-up (partial-failure handoff thread): argued, not changed - upstream handed off in each handler after every successful write, so partial failures and access-check early returns drained the queue; the deferral keeps that condition and moves only the timing. Two fidelity tests pin it (partial failure still hands off; access-check early return still hands off). Execute specs 31 passed.a83a105ff- Stryker-directive sweep: removed the seven// Stryker disable next-linecomments carried in Task.ts's streaming/telemetry region (per the updated AGENTS.md: directives are removed when their file is touched; comment-only, no behavior delta). Equivalence notes for any mutants that now survive there (mutation-diff is advisory):event.nameis always a string on those paths;streamingKeycan never be undefined there; thegetStreamingKeyfallback region is unreachable defensive code. eslint/tsc/Prettier clean, Task spec 180 passed.Test-only commit: the early-return handoff test now asserts both that no whole-patch checkpoint is saved and that the handoff occurs exactly once, per the review thread (negative control: duplicating the finally-block handoff reddens exactly that test). eslint/tsc/Prettier clean.
Refreshed onto upstream/main (tool-call state scoping): ToolCallbacks gained a required toolCallId; the execute spec's callback fixtures pass it (test-tool-call-id, matching main's own spec convention). No production-code change. Full-src eslint 0, tsc 0, execute spec 25 passed, Task spec 191 passed.
Test-only commit: the in-place update test previously asserted nothing; it now asserts no error handling ran and exactly one whole-patch checkpoint with the standard arguments (negative control: skipping the default-on save reddens it). eslint/tsc/Prettier clean, spec 25 passed.
Persistence Integrity walkthrough row - evidence-grade refutation
The flagged move handling is unchanged upstream code, not a regression introduced here:
fs.unlink(source)/ return-success sequence (ApplyPatchTool.ts lines 480-484) is attributed bygit blameto upstream commit0db51d8bf(2026-03-03); this PR's diff against upstream/main contains no hunk touching the unlink region.src/core/task/Task.ts:2400void this.checkpointSave(false, true)), so a locked source already produces a checkpoint containing both source and destination before this PR.Making moves transactional (destination removal, source restore, whole-patch rollback, delayed handoff) changes upstream move semantics repo-wide and is out of scope for the checkpoint-granularity unit; registered as a follow-up candidate for maintainers.
Persistence Integrity row (04:13 walkthrough) - implemented as demanded
destination write is removed (pre-move state restored; a failed
restore is logged), the failure is reported through the tool error
result, and the whole-patch checkpoint is no longer recorded over a
duplicated file.
operations retain their writes, the failed operation reports itself,
and before the queued-message handoff an explicit partial-state
checkpoint persists the retained writes (the access-check failure
path routes through the same branch). The whole-patch checkpoint is
still recorded only after every filesystem operation succeeded.
red; disabling the partial-state branch made exactly the three tests
asserting it red; both restored byte-exact.
Upstream refresh (this push)

The PR diff remains unit-only; no source hunks were altered by the merge.CR thread disposition (handler-error retained writes)
CodeRabbit inline thread (Minor, quick win): when a later file operation rejects outright after earlier handlers succeeded, the outer
catchskipped both checkpoint branches while thefinallyblock still handed off queued messages — the queued turn could start with the retained writes carried forward without rollback metadata.Fix (this push): the
catchnow persists an explicit partial-state checkpoint beforehandleErrorand the handoff whenhandoffPendingis true, with the same default-on semantics as the success/partial branches (skip only whenperWriteCheckpointsis explicitly false; save failures stay suppressed so the original error still reports). Test:persists a partial-state checkpoint when a later handler rejects outright after an earlier write. Negative control: removing the catch-block checkpoint turns exactly that test red (1 red, the new behavior).Review follow-ups (row 10)

ApplyPatchTool.ts:491-510captures the destination content before either supported write path and REFUSES the move before writing when the destination exists but cannot be inspected (non-ENOENT read error).fs.rm(moveAbsolutePath, { force: true })... which deletes the prior destination contents" - false at this head: the unlink-failure rollback (:528-545) restores the captured destination content viafs.writeFileand callsfs.rmonly when the destination was confirmed ABSENT before the write (destinationBackup === undefined), so prior destination data is never deleted by the rollback.applyPatchTool.execute.spec.ts"restores the pre-move state and fails the operation when source deletion fails" (:150), "restores an existing destination when source deletion fails after the move clobbered it" (:194 - the exact clobber case the row claims is unhandled), "refuses the move before writing when the destination cannot be inspected" (:236). The row's cited :149-180 is the absent-destination test; the clobber-restore test at :194 is not acknowledged.Verdict: STALE (CR re-anchored pre-fix hunks). No code change; registration recorded here.
