This pull request updates the plugin to the format specified by reveal.js 4.0.0.
It also fixes a bug with regards to notes and shuffling - previously, the shuffle could result in some of the tags being shuffled into the
Element, as the innerHTML lines were shuffled. The new version removes any child elements of the slide that are an Element or have the .notes class, then appends them again after the shuffle, thereby ensuring that all tags are still present and the speaker notes are preserved.
