Fixes #703.

Summary

  • Strip Pandoc-style explicit heading anchors from rendered Docs markdown headings.
  • Rewrite matching same-document markdown links to native Google Docs heading links.
  • Cover whole-document markdown import, tab/local markdown writes, append, update, replace-range, table-cell links, valid Pandoc IDs, duplicate slug accounting, and scoped rewrites.
