Updates the LoopQuest plugin to 0.2.0.

What changed

• The Module dropdown now lists all current review games — swiper, versus, sorter, detective, fixer, redact, grounding — and drops decoy/arena, which are not ingest modules.
• Added Grounding (claim + source), Sorter buckets, and Mode (gate/monitor) + timeout + on_timeout for blocking human-approval gates.
• The Create Review Task tool now shapes the payload per module, so it works without hand-written JSON.
