   All packages test green (except intermittent worktree pack metadata under npm@latest, being hardened) but npm publish returns PUT E404:

   npm error 404 Not Found - PUT https://registry.npmjs.org/@cobusgreyling%2floop-audit
   … could not be found or you do not have permission to access it.
   

   Provenance signs via OIDC, then the registry rejects the package PUT. Classic NPM_TOKEN was removed from workflows because invalid tokens override OIDC.

   1. On npmjs.com for each package: Settings → Trusted Publisher → GitHub Actions
      • Repo: cobusgreyling/loop-engineering
      • Workflow: exact filename e.g. release-loop-audit.yml
      • Also add trusted publisher for new packages loop-sandbox, loop-swarm
   2. Or create a fresh Automation token with publish on @cobusgreyling/*, set repo secret NPM_TOKEN, and re-add it only to publish steps if OIDC still fails
