This PR removes the
pm-21821-provider-portal-takeoverfeature flag, which is on in Live, enshrining Provider Portal disablement forunpaidprovider subscriptions.Additionally, it removes a manual disablement mechanism via applying metadata to a provider's subscription directly since that is no longer used.
