Fixes #9214.

Description

The text below was written with AI assistance (Claude and GPT-6 Astra).

With ignore_index set, FocalLoss(reduction="mean") divided the summed loss by mask.sum(). The mask from create_ignore_mask has one channel, while the loss has one value per voxel and channel, so the result was the masked mean multiplied by the number of loss channels (C, or C-1 with include_background=False). I now count the mask once per loss channel with mask.expand_as(loss), the same way AsymmetricFocalLoss does. The "sum" and "none" reductions and single-channel losses do not change.

I found this by reading and testing the code.

Case Before After
Reproducer from the issue (4 channels, no voxel ignored): FocalLoss() vs FocalLoss(ignore_index=255) 0.306783 vs 1.227131 (4.0x) 0.306783 vs 0.306783 (1.0x)
MNI152 2 mm T1 + 6-label structural segmentation from MONAI's test data, small UNet logits, no voxel ignored 0.294406 vs 1.766435 (6.0x); gradient norm 6.0x 0.294406 vs 0.294406; gradient norm 1.0x
Same volume, top 40% of axial slices set to 255: FocalLoss(ignore_index=255) vs the same loss on the labelled slices only 1.795147 vs 0.299191 0.299191 vs 0.299191
Same, DiceLoss(ignore_index=255) + FocalLoss(ignore_index=255): focal share of the total 0.668 (0.251 on the labelled slices) 0.251

The new test test_focal_loss_masked_mean_scale in tests/losses/test_ignore_index_losses.py checks that ignoring two rows gives the same mean as cropping them away, for softmax, sigmoid and include_background=False. All three cases fail before the change (3x, 3x and 2x off) and pass after it. test_ignore_index_losses.py, test_focal_loss.py, test_dice_focal_loss.py and test_generalized_dice_focal_loss.py pass (78 tests). Black, isort and Ruff pass on the changed files.

Types of changes

  • Non-breaking change (fix or new feature that would not break existing functionality).
  • New tests added to cover the changes.

I made this PR and directed its development, with AI assistance from Claude and GPT-6 Astra.

I found that FocalLoss with ignore_index and the default mean reduction divided the summed loss by the number of valid voxels rather than valid voxel-channel elements, so the loss and its gradients grew by the number of channels. I now count the single-channel mask once per loss channel, as AsymmetricFocalLoss already does, and add a test that ignoring voxels gives the same mean as cropping them away.

📝 Walkthrough

FocalLoss mean reduction now expands the valid-element mask to the loss shape before counting the denominator. A parameterized regression test compares loss with ignored elements against loss computed on the remaining elements.

Explanation

Issue #9214 requires the masked mean to match the mean over retained voxel-channel elements. FocalLoss now uses mask.expand_as(loss).sum() for the mean denominator. The added regression test covers softmax, sigmoid, and include_background=False. The sum and none reductions remain unchanged.

Document the test parameters.

Add a Google-style Args: section for use_softmax and include_background. The current docstring describes the assertion but not the parameterized inputs. As per path instructions, “Docstrings should be present for all definition which describe each variable, return value, and raised exception in the appropriate section of the Google-style of docstrings.”

Cover an unused ignore_index in the multichannel case.

The test always sets part of target to 255. A regression that changes only the configured-but-unused ignore_index path could pass it. Existing no-ignore coverage uses ignore_index=None, and the other mean test uses one channel.
