Refactor image reading

Description

Some cleanup ahead of more refactoring.

• Change default batch size to 25
• Remove some of the options from the custom batch size
• Fix right alignment of links
• Fix back link
• Include screening date in status bar

## Image reading refactor

Significant refactor of much of image reading, including:

• Try to simplify templates and separate concerns
• Add concept of being in a reading workflow
• Suppress nav when in workflow
• Add review page for all opinions except normal
• Add existing-read view when returning to a case
• Add medical information summary from screening
• Fix misc bugs
• Start cleaning up data structure
