Description
After discussions with UCD team, updates the incomplete mammography question with revised text and adds a new checkbox for imperfect images.
Initial screen
Collecting items under a new 'Additional details' heading
Imperfect images checkbox
Allows a mammographer to acknowledge the images are not of optimal quality but let the image reader know they were the best possible images given the circumstances.
Not all mammograms taken
Collects a set of reasons with optional free text, and asks if another appointment should be scheduled.
Recall appointment
If another appointment can be helped, we can collect details to be shown to the admin staff booking it
