Updated the binding strength for Observation.code to 'preferred' and removed several elements related to coding slices and references.
This is due to other nations not using PaLM or PBCL, so the slicing is not required. Instead this will be created in a NHSE profile. Note that default ValueSet is PaLM, this was agreed in C&TA 7
