Process documentation template
1. Process Title
Name of the Process: A clear, descriptive title.
2. Purpose
Objective: Briefly describe the main goal of the process. What is the process intended to achieve?
3. Scope
Inclusions: What does this process cover?
Exclusions: What is explicitly out of scope for this process?
4. Process Requirements
Tools and Resources: Identify any tools, resources, or systems required to execute the process.
Prerequisites: Conditions or items that must be in place before the process can start.
5. Procedure
Steps: Numbered list of steps in the order they should be performed. Include the following for each step:
Decision Points: Any branches in the process where decisions must be made, and who makes them.
7. Process Flow Diagram
Visual Representation: Include a flowchart or diagram to visually represent the process flow. This is especially helpful for complex processes.
8. Compliance and Standards
• Regulatory Requirements: Any legal or regulatory requirements that impact the process.
• Internal Standards: Any organizational standards or best practices that the process aligns with.
9. Risks and Controls
• Potential Risks: Identify risks associated with the process.
• Mitigation Strategies: Controls and strategies to mitigate identified risks.
10. Review and Improvement
• Review Schedule: How often the process will be reviewed and by whom.
• Continuous Improvement: Mechanisms for suggesting and implementing improvements to the process.
11. Appendices
• Supporting Information: Additional information or documentation that supports the process.
• Link to project Glossary: Update definitions of terms used in the document in the project glossary.
