signalfx_org_token double url-encodes names when trying to read from previously created state. #341

Creating a token named "test/token" works properly, but when refreshing state it mistakenly tries to read the URL
https://api.us1.signalfx.com/v2/token/test%252Ftoken but should be https://api.us1.signalfx.com/v2/token/test%2Ftoken
