[Bug]: File-change approval prompt shows no filename or change details #5255

Steps to reproduce

  1. Open a Git repository in T3 Code using the Codex provider.
  2. Start a thread with tool approvals enabled.
  3. Ask Codex to perform a task that creates and then deletes or modifies a temporary file. In my case, it created /private/tmp/velos-unassigned-alert-issue.md to submit a GitHub issue, then attempted to remove it.
  4. Wait for the file-change approval prompt.
  5. Observe that the approval panel says “File-change approval requested”, but does not show the filename, path, diff, or any description of the requested change.

Expected behavior

The requested command should be displayed underneath the "PENDING APPROVAL" heading
