How does the app work? (Does it make reg entries? Inject into Explorer memory? Completely replace the OS contextmenu?)

Just wondering, on a broad level, how the app does its job. Does Shell have to be running constantly in the background in order to work? Does it use memory injection to modify the OS context menu? Does it disable the OS context menu and replace it with a similar but customizable one?
