What would be the reason lens icon and ... always appear in resource listing?

I don't think this is a bug, because the issue is on a project upgraded from nova 3 to 4. I am trying to understand why these icons appear even there is no lens or any action defined at the resource level

The policy for this resource is:

I added also the method

  public function replicate(User $user, Country $country)
    {
        return false;
    }
    

To make sure no replication is authorized, but without effect.

I am missing something on the update, but now sure where to look.
