userMenu from Tool overwriting the NovaServiceProvider

Description:

If you try to add a MenuItem to the userMenu from a Tool, it will overwrite the userMenu made in the NovaServiceProvider.

Detailed steps to reproduce the issue on a fresh Nova installation:

Create a Tool: php artisan nova:tool acme/price-tracker
In the Tool's /src/Jetstream.php boot method, add a UserMenu Item:

            $menu->append(
