Clean up Forevr Profile

Tighten up the profile display. This is how the iPhone does the screen for entering a contact:

I wish I could get the prepends to be the same width, but adding spans makes it not responsive.

I also with there were button groups, but I don't see that option. I'd group phone and email.

I also think that we could make it so you could change the labels and add a new field, but that's for later.
