I have updated the version numbers and pushed a PR to jaredpar:master.

@jaredpar Can you please accept my PR to upgrade to VS2019?

Also, please update the extension on the Marketplace when you can.
I've uploaded a release of v1.0.3 to my fork. Feel free to upload that one, to save yourself having to build it locally.
