Massive usage bug in 1.0.21

I made 28 premium requests to ChatGPT 5.4, but it counted as nearly 300! It was draining as the session was ongoing. When I ended the session, it showed the correct amount (28 requests), but on my GitHub usage, it showed nearly 300 requests. This was on a GitHub Business account. Switched over to opencode and that is working correctly.
