Extensions won't activate

I'm hosting code-server from a SageMaker notebook instance. I followed this guide (option B) for the setup. Everything was working fine until a couple of days ago when suddenly extensions wouldn't activate any longer. The weird thing is that all extensions with activationEvent: '*', load fine but nothing else.
I tried installing extensions manually but same result.
Not sure this is a bug or I just need to tweak a setting. Any help would be much appreciated.

Hmm this is definitely a tricky one. Do you see any logs in the browser console?

Hmm...I wonder if it's that "Refused to display" message 🤔 @code-asher any tips for helping them debug this?

I was thinking it might have more to do with the web worker extension host not starting. But not sure. I don't understand why the extensions that load at startup are fine, but every other activation event fails.

Coincidentally the extensions that do load are git related and enabled in jupyter_notebook_config.json (but could be totally unrelated):

The web worker could be related, I think if the web worker extension host dies Code will also kill the server extension host for some reason (or at least the last time I looked it was this way but it might no longer be).

Could you set X-Frame-Options to sameorigin to see if the problem persists?

The issue now is that when I try to open a .ipynb file it gives me this error which I'm trying to resolve:

Were you ever able to resolve this issue? I'm encountering the same thing and was hoping you had managed to find a solution.
