Add Windsurf Issue Triage example doc

Goal

Add a Windsurf Issue Triage example. Pattern coverage currently leaves Issue Triage × Windsurf empty.

Files

• examples/windsurf/issue-triage.md (new)
• examples/windsurf/README.md (link it)
• examples/README.md (fill Issue Triage × Windsurf cell)

Acceptance criteria

• Week-one: propose labels/priority only — no auto-apply or close
• State file schema matches issue-triage-state.md pattern
• Safety: report-only first week; human gate before label writes
• Follow tone of examples/cursor/issue-triage.md
