Scrolling in Modal dialogs: modal action hidden #5099

Description

In long modals, the need to scroll obfuscates the heading/close button and the CTA buttons.

Some places we're seeing this:

Add a form element Preview extends beyond "Add form element" window forcing horizontal scroll #3910
Generating an API Key Modal design: Need to scroll to see top/bottom of modal with important user actions #4728
Downloading responses from the vault Modal action hidden, requires scrolling #2923
Confirming responses from the vault Endless scroll when confirming responses (pasting receipt codes) #3639

Expected behaviour

Consider freezing the frame with the heading/title/close button statically locked and the CTAs also always visible
