One OpenCode session's modified files appear in another session's `Modified Files` list in the same repo

Description

Is it an expected behavior? The problem is that, when I revert the message in session B but session A is currently making changes in the same repo and the same branch, reverting session B will also revert changes by session A.

1. Start 2 cli tabs with 2 opencode sessions
2. Make changes in session A and chat in session B at the same time
3. Revert messages in session B
