Display version numbers in output

In the generated XLSX files (query and annotate), adopt a line at the top that displays information about which versions were used:

• SASTA version
• Method category + version (method.name)

Don't include versioning in the forms for now, this will have to be done in consultation with Jan Odijk.

Optional: Also adopt at which date and time the file was generated.

Output works, but the version number has to be updated manually in settings.py
