[Forge 1.16.5] All players appear to have OP Rank (but actually don't)

For some weird reason, the "OP Rank" is being given by the server to every player. I have just installed the Server Utils mod on my server a few days ago. There should only be about 4-5 OPs, however it seems that everyone is being given the rank as a default.

I am unsure of how to provide reproduction steps for this bug, it seems to be incredibly anomalous behavior. However, if it helps to provide this information, all the players that have joined the server since Server Utilities was installed have previously joined before the mod was installed. I don't know if this has anything to do with things, but I figured I would mention it anyway.

Not sure what else to say, so I'll end this with "please help."

Update: It appears to be purely a visual issue but I think it's one that still needs addressing.
