Request - make column headers for locality attribute tools consistent

Describe what you're trying to do

I've been bulkloading some locality attributes and the column headers for the tool differ from those in the main bulkloader.

locality attribute bulk unloader	attribute_type	attribute_value		attribute_determiner	determined_date		attribute_remark

I suggest the following changes for the bold terms

determined_date - change to attribute_date
determination_method - change to attribute_method

This will make the column headers for the various tools more consistent. If possible, it would be nice to put them in the same order in the tools as they are in the main bulkloader.

I cant seem to find any active requests that are similar to this (change bulkload tool header)--- maybe there have been addressed. This makes sense but we'll review on Monday
